As a community nurse, you would be supported to develop into an autonomous practitioner nursing patients with multiple complex health care needs in their own home.

Main duties of the job

To contribute to processes for service and quality improvementAssess the physical and psycho-social needs of patients and delivertherapeutic interventions based on best available evidence in order toimprove health outcomes.To manage own case load, delivering and evaluating service delivery topatients with a variety of medical conditions and those delegated by casemanagers.To plan interventions with the Case Manager and other team membersusing the common assessment processTo take part in the referral management process to ensure patients aretreated by the most appropriate member of the team/serviceTo contribute to the development of protocols and procedures developedfor clinical pathwaysAssist in the development of case management protocols and guidelineswithin their own area of professional practiceParticipate in weekend rotas / 7 day working rotas

EDUCATION, QUALIFICATIONS & TRAINING

Essential

  • Registered Nurse or student nurse awaiting confirmation of NMC registration


Desirable

  • Post registration education and extended skills eg Intravenous drug administration , compression dressings Desirable but not essential
  • Post registration education and extended skills e.g. Intravenous drug administration, compression dressings Desirable but not essential


SKILLS, ABILITIES & KNOWLEDGE

Essential

  • To be able to communicate effectively with patients, relatives, carers and other professionals.
  • To be able to care for patients without prejudice related to gender, religion or disability.
  • Able to work under pressure
  • Ability to manage own Caseload
  • Knowledge of the NMC Code of Professional Conduct
  • Knowledge of the Data Protection Act


EXPERIENCE

Essential

  • Demonstrate experience post or pre-registration placements in relevant clinical areas: ? Care of Elderly ? Community ? Orthopaedics ? Neurology ? Respiratory ? Palliative Care ? Long term conditions


Desirable

  • Knowledge of working with a single assessment, planning and the delivery of care
  • Experience of electronic clinical systems and technology e.g. RIO


SPECIAL CIRCUMSTANCES

Essential

  • Car driver / owner able to travel to other locations within the trust
