Job summary

We are seeking a Senior Mental Health Practitioner - Nurse/ Social Worker or OT to join the Southampton Assertive Outreach Team. We are one of the last A.O.T. teams in the country and have a proven track record of working successfully with this challenging client group. The successful applicant will provide care coordination for a caseload of up 15 clients with complex needs and interventions to service users with severe or enduring mental health needs across the city. AOT is an interesting and exciting place to work. We are a multicultural community with many areas of high deprivation and we also have two universities on our patch. The post involves a lot of multi-agency working and the successful candidate will work closely with members of the team.

We are looking for an enthusiastic and creative person who is committed to working with a diverse community and to maximising life opportunities for our clients and their families. The work is varied and challenging, and there is a strong team approach ensuring good support for the successful applicant. You will need to have a flexible, multi disciplinary approach and have a solid understanding of risk assessment, care planning and the recovery model. You will also need to be able to offer support and advice to less experienced colleagues. Some knowledge of either psycho-pharmacology or Psycho-Social Interventions would be advantageous.

Use of a car is an essential requirement of the post.

The role is to provide good quality assessments and interventions to service users who have severe or enduring mental health problems. Support should also be offered to their families and carers.
