Luton Sexual Health is a fully integrated sexual health and HIV service based in the community in Luton town centre. There are approximately 750 people accessing HIV care in Luton, from a wide range of backgrounds. HIV services are delivered in person and via telephone consultations and comprehensive sexual health care is provided in a fully integrated manner.

This role is for a nurse with experience working with people living with HIV to work as an integral part of the HIV MDT at Luton Sexual Health.

The MDT comprises:

HIV/Sexual Health Consultants (3.4 WTE)

HIV Specialist Pharmacist (1 WTE)

HIV/SH Pharmacy Technician (0.6 WTE)

HIV/LGBT+ Clinical Psychologist (1 WTE)

Health & Wellbeing Advisor (1 WTE)

Admin assistant (1 WTE)

There are 4 consultant clinics delivered per week and 3 Specialist Pharmacist clinics. The MDT meeting is held every Friday at 2pm, with additional quarterly HIV-GP and HIV-Maternity MDT meetings on a quarterly basis.

Main duties of the job

  • Provide HIV prevention, treatment and care as part of a fully integrated sexual health and HIV service
  • Deliver PrEP clinics for assessment, initiation and continuation of PrEP for clients considered to be at risk of HIV transmission.
  • Provide expert nursing input to a cohort of clients living with HIV, providing treatment, care and support within the services.
  • Act as an integral member of the MDT, contributing to care discussions and treatment decisions, and relaying information to clients.
  • As part of a nursing team the post holder will take a lead role for HIV, reviewing outcome data, developing guidance, and identifying areas for change
  • Practice autonomously, working independently within scope of clinical practice and in conjunction with other relevant clinical staff
  • Manage a caseload of patients both in face to face and telephone clinics, delivering individualised and personalised direct patient care
  • Support the development of staff caring for patients through support, formal education, practice development and role modelling.
  • Be professionally accountable for the maintenance of standards and professional practice as set by Trust Policies/Procedures, the NMC Code of Professional Standards of Practice and Behaviour and relevant professional guidelines

Staff are responsible for complying with the relevant standards set by their Line Manager. A breach of such standards may lead to disciplinary action. It would be investigated fairly and appropriate steps taken to prevent a recurrence and address any wider causes.

Clinical duties and responsibilities
  • Receive referrals via telephone, email, letter and other forms of direct patient referral. Triage case load according to urgency, agreed protocols and working practices.
  • Participate in nurse-led and multidisciplinary clinics, ward rounds, patient reviews and multidisciplinary team meetings, co-ordinating treatment independently or in conjunction with a wider multidisciplinary team.
  • Where needed conduct domiciliary visits for review, assessment and sample-taking.
  • Prescribe antiretroviral and other relevant medications within scope of practice and provide advice on medicines management issues related to medicines within scope of practice
  • Use skills in history taking, examination and investigations to assess the health and well-being of clients.
  • Document clearly in paper based and electronic systems, ensuring all entries are accurate, legible, timely and in accordance with Trust standards
  • Provide a high quality service from diagnosis/referral through to assessment, treatment initiation and long-term follow up, referring to other specialists as required. Provide guidance to staff, patients, their families and carers on pathway navigation.
  • Communicate with clients/patients and their relatives/carers, GPs and other health professionals via telephone, email and letter, provide reports and liaise as required with medical staff and other members of the multidisciplinary team both verbally and in writing.
  • Contribute to the active engagement of service users through facilitation of client, carer and stakeholder feedback via user groups, open days, support groups. Ensure feedback informs service development
  • Act as an advocate for clients to ensure an individually focused approach to the delivery of care and service strategy .Support and enable clients and carers to make informed decisions relating to their treatment and management
  • Provide non-judgemental and holistic care, having regard to individuals culture, beliefs, gender identity and sexual orientation.
  • Escalate concerns within multidisciplinary teams / management colleagues as required, maintaining an awareness that ethical dilemmas may require discussion through an agreed / defined process.
  • Advise on risk reduction, health promotion and prevention of illness, escalating concerns as required to appropriate members of the multi-disciplinary team.
  • Identify, report and raise concerns about Safeguarding via agreed protocols within Trust guidance. Maintain familiarity with Trust policies, procedures and systems for safeguarding including the assessment of Mental Capacity, the process for consideration requests for Deprivation of Liberty Safeguards (DOLS) and the application of the principles of Prevent (counter terrorism awareness).

Governance, service improvement and educational responsibilities
  • Where required be accountable for the management of incidents and have responsibility for the processing of reports as per Trust Policy and Procedures. Participate in learning from incidents, including provision of feedback, reflection and action in practice.
  • Participate in local, regional and national research trials in conjunction with the MDT. Maintain up to date Good Clinical Practice Certification
  • Participate in the development and delivery of patient experience initiatives, seeking innovative ways to seek patient experience and feedback in relation to service. Address any immediate concerns or complaints as they arise and escalate as required. Where required respond to concerns, complaints or feedback within agreed timescales.
  • Develop links and collaborative working relationships which enhance the interface between staff, patients, community and stakeholders across organisational and professional boundaries.
  • Attend and where required chair multi-disciplinary meetings and forums as a representative of the service. Ensure minutes and actions are recorded, agreed actions are undertaken according to timescales set.
  • Act as an expert educational resource for clinical staff, patients carers and relevant external agencies by providing formal and informal education.
