The post holder will undertake assessments in respect of NHS Continuing Healthcare & NHS Funded Nursing Care. The post holder will assist in the local implementation of the National Framework for NHS Continuing Healthcare & NHS Funded Nursing Care and any subsequent guidance. The Post Holder will participate in the delivery of multi-agency training in respect of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care. The post holder will undertake assessments in respect of NHS Continuing Healthcare & NHS Funded Nursing Care.
Main duties of the job
The post holder will advise on issues relating to the assessment and implementation of complex home care packages, liaising with the patient/service user, their representatives, and relevant health and social care professionals. The post holder will assist with local implementation of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care. The post holder will negotiate with care providers, both NHS and external agencies in respect of how care will be delivered to individual service users who have highly complex, and / or intense health care needs and then apply for appropriate funding as required. The post holder will undertake assessments of service users being placed in nursing home to identify the correct funding in respect of their care. The post holder will co-ordinate and 'chair' multi-disciplinary team meetings where complex and often sensitive information is evaluated and service users will be assessed for eligibility of NHS Continuing Healthcare and NHS Funded Nursing Care. The post holder will contribute to the formulation and if necessary co-ordinate a plan of care to meet the needs of service users following individual assessments. The post holder will, if requested to do so provide clinical advice and support to staff working in the clinical support unit.

The post holder will ensure all equipment for service users funded and provided by Tower Hamlets Clinical Commissioning Group is maintained in accordance with manufacturers warrantee and Health and Safety guidance. The post holder will implement the system of informing service users and service providers of different funding bands to ensure complex, sensitive and sometimes controversial information is appropriately translated and communicated effectively. The post holder will be responsible for ensuring that accurate and comprehensive verbal and written reports relating to information regarding people placed in nursing homes are provided when required. The post holder will inform appropriate line managers at ELFT in respect of issues relating to poor standard of care for service users placed in nursing homes and where required take necessary action including multi-agency working with regulators and social services. The post holder will co-ordinate and provide both verbal and written reports in respect of service users requiring their total package of care to be fully funded by Tower Hamlets Clinical Commissioning Group. The post holder will, participate and contribute to the delivery of the established training programme for multi-agency workers in respect of NHS Fully Funded packages of care. The post holder will promote a culture of best practice using evidence based research.

The post holder will analyse information obtained during assessment of service users funding needs and provide it in a format that could influence actions taken by Tower Hamlets Clinical Commissioning Group as a result. The post holder will evaluate and analyse the information obtained during comprehensive assessments, which can often be complex and sensitive. The information should be translated in an easily understood format and used to establish individualized plans of care for service users. The post holder will contribute to the gathering of relevant information required for effective use of databases. - 1st Level Registered Nurse (RGN/RMN)
- ENB Certificate 998 Teaching and Assessing/ Preparation for Mentorship Qualification or equivalent qualification
- Good knowledge and proven demonstrable experience of delivering 'hands on care' care within the community setting
- Must be able to demonstrate experience in audit and monitoring of services and managing performance of services.
- Good working knowledge and evidence of implementation of Gold Standard Framework, Recognised End of Life Care Pathways and Preferred Place of Death, Six Steps initiatives in the community setting
- The post holder will undertake assessments in respect of NHS Continuing Healthcare & NHS Funded Nursing Care.
- Able to demonstrate experience of effective team leadership, management of change, and an ability to work independently but also as a team member
- Evidence of Sound IT Skills
- Able to produce concise and accurate verbal and written reports
