CLCH is working in partnership with an alliance of local providers (NHS acute, community and primary care providers, 2 Local Authorities, and a variety of Third Sector organisations). CLCH is looking for a Band 8a senior nurse who is dynamic, is a creative thinker, is collaborative and works as a team player who understands how to adapt to new situations and is passionate about improving health and social care through integration.

The local system across West London is on an exciting journey to improve the way that health, social care and the third sector services work together. The local system has co-produced its Integrated Care Strategy and is now working together to deliver an Integrated Community Team approach.

The team works collaboratively with patients, their families and carers to ensure that patients receive the necessary support to understand and manage their condition themselves, wherever possible, with a clear treatment plan and information on the range of services available and how to access them.

Main duties of the job

This senior case manager role will be key in the Integrated Community Team and will focus on patients with complex needs.

The Integrated Community Team (ICT) model for West London aims to deliver more consistent care for patients in a variety of settings depending on the needs of the patient. This includes care delivery in the community, in patient's own homes, in the GP practice, and within two multi-functional Integrated Care Centres at St Charles and Violet Melchett.

The Senior Case Manager role offers a new and exciting opportunity to become part of the ICT which bringing together health and social care expertise to deliver holistic integrated care for patients in West London CCG. This holistic approach to providing care to people with complex needs includes the following key functions:
  • Risk Stratification and managing risk.
  • Holistic assessment of health, social and wellbeing needs
  • Personalised Care Planning
  • Care delivery and care
  • Prevention and Self care
  • Patient centred multidisciplinary team

The role will involve both care coordination and care delivery and aims (where appropriate) to prevent duplication and minimise the number of professionals involved in the care of the patient.

The post holder will be a key member of the team whose overarching goal will be to work collaboratively to provide improved and more effective integrated care that deliver better outcomes and experiences for patients and their carers.
