Our Intensive Support Team are looking to welcome a Registered Learning Disability or Mental Health Nurse (or other registered nurse with relevant experience) to join us in providing a specialist service to adults with a Learning Disability in Portsmouth City!

This is a an rare opportunity to join our multi disciplinary Intensive support team, part of our 'Outstanding' (CQC Rated) Integrated Learning Disability Service.

We work to meet the needs of patients and their families to achieve improved health outcomes and promote health and wellbeing while demonstrating care, compassion, competence, communication, courage, and commitment.

Our dedicated team have a passion progress and develop our service by
  • Improving access to mainstream healthcare services
  • Supporting the development of enhanced, or adapted, mainstream healthcare services
  • The provision of appropriate information to Service Users and their carers.
  • Facilitating the increased involvement of Service Users in healthcare services


Main duties of the job

*To work as part of the Intensive Support Team (IST) in providing high standards of effective health care for people accessing the outreach in their own homes or enhanced supportive living services.

*To work as part of the Intensive Support Team in conducting an holistic approach to assessment and intervention, with the nursing role involving the undertaking of physical and mental health assessments and intervention.

* Contribute to the development of a Positive Behaviour Support (PBS) plan. The nurses main contribution with relate to physical and mental health needs

*To role model, train and support staff in the implementation of the PBS plan including the physical health and mental health aspects

*To monitor and support complex healthcare packages that are responsive to changing health needs, (e.g. around challenging behaviour, mental health needs and complex physical health).

*Assess patients /clients, plan develop/implement/evaluate programmes of care and carry out nursing procedures.

*To contribute to Stopping the Over Medication of People with a Learning Disability and/or Autism (STOMP) by working in partnership with other disciplines to reduce or withdraw psychotropic medication that has been prescribed in the management of 'challenging behaviour'

Person Specification

Qualifications

Essential

  • RNLD or RGN / RMN (with appropriate experience and knowledge) or equivalent essential (other parts of the Nursing Register may be considered dependent upon relevant experience) currently registered with NMC.
  • Teaching Qualification, e.g. ENB 998 or equivalent
  • Full clean driving licence


Experience

Essential

  • Experience of working with people with a learning disability within a range of learning disability service settings and in response to a variety of needs.
  • Experienced in developing and delivering care and clinical interventions to patients
  • Experienced in assessing and developing patient care plans and in developing and delivering care packages.
  • Experience of MDT working
  • Experience of teaching, trainingand supervision
  • Evidence of recent CPD
  • Evidence of developing policy, guidelines contributing to management of resources
