Job summary

Harrow Community Nursing service is seeking to appoint a professional, competent, enthusiastic and skilled colleague to join us to build on existing models of care and introduce new ways of working. The transformation plan over the next 12 months includes further integration across primary and acute care, local authority, voluntary sectors and patient groups, with a greater emphasis on population health through collaborative working with neighbourhood teams

The successful applicant will be passionate about improving quality of care and ensuring equity of service provision to the residents of Harrow. We are therefore looking to appoint a leader who reflects the core values of the Trust

The post is for a motivated and committed Integrated Locality Manager (Band 8A) to join our Integrated Care Service in the Borough of Harrow.

Main duties of the job

The appointee will be responsible for the operational and clinical management of the patient care services within Harrow Community Nursing Team. They will be expected to provide strong clinical and operational leadership to staff and patient care in accordance with Community Adult Health Services clinical model. The post holder will focus on the continuing development of a seamless integrated service between the primary, acute and community Trusts as well as other partner organisations.

The post holder must have the ability to function autonomously and in collaboration with other Trust services and external service providers.

Applicants should have leadership and management experience, and excellent communication skills.

This is an exciting role with responsibility for performance, quality and transformation within the Harrow Integrated Care Clinical Business Unit. The appointee should have a proven track record of management and delivery of operational services with creativity. Creativity is expected to redesign services to meet the changing needs of the population and integration with Partners.

Job responsibilities

  • The post holder will work as a minimum, 20% of their time in direct patient care. This will be spent supporting caseload allocation, providing practice development and education and competence support to the Community Nurses.
  • In exceptional circumstances respond directly to patient need for urgent response
  • The post holder will maintain expert clinical competency, support the Clinical Business Unit Manager in policy development and implementation across the locality, contributing to the modernisation of urgent care and the development of the case management model across the health econom
  • Work with the primary care cluster in the identification of those patients at high risk for acute hospital admission and ensure those identified are assessed and have care planned appropriately to reduce their risk of admission.
  • Empower patients and carers through appropriate support and education, ensure care provided and service delivered are in line with National Service Framework, NICE Guidelines, Trust policies and procedures and the Clinical Governance Framework
  • To work with the Community Nurses on the promotion of patient self-management programmes within the locality
  • To act as an expert clinical resource to the locality, working clinically as required.

  • Registered Nurse
  • Degree/ masters level qualification relevant to community practice or Evidence of recent CPD
  • Post-graduate management and/or leadership training
  • Clinical supervisor training
  • Specialist Community Practitioner/District Nursing qualification
