We are looking for a skilled and highly motivated Learning Disability Nurse/Practitioner with a passion for delivering the 6 Cs (care, compassion, commitment, courage, communication and competence)to join the North and Mid Intensive Assessment and Treatment Team (IATT) based in Tiverton in Mid Devon.

North and Mid IATT supports people with learning disabilities who have a wide range of health issues. The clinical work is varied and dynamic, involving a range of complexity. As a Learning Disability Nurse / Practitioner, you will be part of our multi-disciplinary team.

This is an opportunity to join a Learning Disabilities Service in Devon, as we develop and improve our service to meet the needs of local people and their families and Carers. We work closely our colleagues across agencies including Mental Health Services, Devon County Council, Primary / Secondary Care and the Third Sector.

Main duties of the job

The purpose of this role is to:
  • You will be working with people across the service area who have a Learning Disability and associated complex health needs.
  • You will work within the team, providing clinical support to adults with learning disabilities who have additional complex needs, which may include challenging behaviour, physical and sensory impairment, mental health and forensic issues.
  • The post holder will play an integral role in supporting the delivery of care within these pathways within a variety of community settings.
  • You will be developing skills in working with adults with learning disabilities, and associated communication and behavioural needs.
  • You will provide assessment and implementation of evidence-based interventions for people with learning disabilities and associated needs.
  • You will support the coordination of care for a specified group of service users.
  • Applicants need to have the ability to travel across the Trust Learning Disability Service area.
  • It is expected you will be able to use IT clinical record systems.
