To act as the lead stroke specialist nurse, working with the Stroke clinicians to assist in the enhancement of stroke services in Aneurin Bevan Health Board, providing expert emergency clinical practice, advice, education, and effective nursing management of people who have experienced stroke, assuring high quality patient care.
Main duties of the job
Effectively manage their own caseload of stroke patients in secondary care, Monday to Friday 9-5pm (or according to the needs of the service) and on a rotation for weekends, within pre-determined pathways, guidance, and protocols. Promote effective working links with both NHS and voluntary sectors to ensure a coordinated specialist service to patients with complex stroke needs, need along with their families/carers. Be required to be involved in the implementation of the collation of audit data, alongside quality improvement initiatives to improve the patient pathway. The core role of the Advanced Clinical Nurse Practitioner will be to autonomously examine, diagnose and treat patients. To promote evidence-based practice and act as an accountable role model.

The post holder will effectively manage their own caseload of stroke patients in secondary care, Monday to Friday 9-5pm (or according to the needs of the service) and on a rotation for weekends, within pre-determined pathways, guidance, and protocols. Promote effective working links with both NHS and voluntary sectors to ensure a coordinated specialist service to patients with complex stroke needs, need along with their families/carers. Be required to be involved in the implementation of the collation of audit data, alongside quality improvement initiatives to improve the patient pathway. The post holder practices advanced nursing within the Framework for Advanced Practice in Wales 2010. Core service delivery is covered by four pillars of practice, Clinical Practice, Management and Leadership, Education and Research. The post holder is expected to monitor standards of nursing care, patient experience and patient safety and take appropriate action to maintain and improve them Ensuring that all delegated activities are in line with the NLIAH (2010) All Wales Guidelines for Delegation and/or HEIW Frameworks. The post holder is required to be registered with the Nursing and Midwifery Council.
