Job summary

We are looking to recruit 2 x Clinical Leads - Community Nurse, working full time 37.5 hours per week and part time 22.5 hours per week (Weekends included and working day between 8am and 5pm included) to join our happy, dynamic Community Care Team.

The post holder will be a clinical resource, providing a visible clinical profile, having advisory, leadership and clinical oversight of the clinical caseload.Working collaboratively with the wider healthcare team to meet theneeds of patients, ensuring patients receive the right care, by the rightperson at the right time and in the right place.Placing the patient at the centre of all decisions, reinforcing theconcept of a more streamlined patient journey, with minimal avoidabledelays, and an emphasis on people achieving independence whilstimproving the quality of care.

**We offer a relocation package of up to �8,000**

The relocation package is for anyone who would need to relocate in order to take up employment with Cheshire and Wirral Partnership.

To be eligible for the relocation payment, you must live more than 40 miles from the base/location of the post you are applying for and you would need to relocate to within 15 miles of the new base.

Main duties of the job

  • To provide direction within the caseload in consideration of the impactof current health and social care agendas and local initiatives
  • To focus on the delivery of person centred care
  • To provide effective, evidence-based care to a defined populationthrough case load management and monitoring of care standards
  • Provide leadership including clinical supervision to team members and provide support to the Care Community Team managerial lead.

All new clinical support staff will be required to have completed or working towards the Care Certificate (CWP In-house 2 day training to be completed within 6 months of start date).

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment.You are encouraged to enrol for the DBS Update Service. A small annual fee of �13 per year applies.

New entrants to the NHS will commence on the minimum of the scale stated above.

Person Specification

Qualifications

Essential

  • First level registered general adult nurse
  • Evidence of teaching and assessing
  • SPC qualification in District Nursing or equivalent; demonstrating robust evidence of caseload management, community profiling and application of health needs assessment


Knowledge and Expertise

Essential

  • Proficiency in delivering high quality evidenced based care.
  • To be able to demonstrate continuing professional development
  • Ability to work and communicate effectively within a MDT setting


Experience

Essential

  • Significant experience of working within a community healthcare setting.
  • Experience of making autonomous decisions
