This is an exciting opportunity for a confident and competent Advanced Clinical Practitioner to work alongside the Community Geriatrician in Rotherham, caring for patients within their own homes and also patients within Intermediate Care rehabilitation beds. This post will work closely with the Community Advanced Clinical Practitioners to help provide an acute see and treat model. The Community ACPs are part of the wider Urgent Community Response Team who respond rapidly to provide the correct clinical interventions are provided in a timely manner. Our aim is to deliver high quality care to help reduce the demand for hospital attendance and admission, working closely with GPs, Yorkshire Ambulance service and other multi agency professionals.

Main duties of the job

With substantial post-registration experience, including the management of acute minor injuries and illnesses, including falls and non-medicatical V300 prescribing. You will use your wide ranging clinical knowledge and skills to assess, diagnose, treat, and prescribe as necessary and should have excellent communication skills.

You will be expected to work towards being an autonomous practitioner across all four pillars of advanced practice. You will have skills to provide advice, support and education to colleagues to improve the quality of care they deliver.

As an organisation we are committed to supporting flexible working including part time hours however as part of the apprentice standard, the minimum contracted hours allowable are 30 hours per week.

Experience

Essential

  • Extensive and Relevant post Registration Experience.
  • Experience of working autonomously as an ACP.
  • Demonstrate experience in a senior post.
  • Experience in multi-disciplinary staff working.
  • Experience of working with Older People.


Desirable

  • Experience of working in a Community Setting.
  • Experience of undertaking triage.

  • Minor Injury Qualification.
  • Evidence of Ongoing Professional Development.
  • Portfolio evidencing competency as an ACP.


Desirable

  • Emergency Nurse Practitioner Qualification (or relevant experience).


Communication and relationships

Essential

  • Able to present and receive complex information within a multi professional team.
  • Ability to convey sensitive information in an empathetic manner to patients, relative/ carers and staff.
  • Demonstrate effective written and verbal communication skills.
  • Produce timely, accurate written/electronic records and documents.
