The aim of the Every Minute Matters (EMM) programme is to ensure that each day in hospital actively supports our patients' progress so that no patient is in hospital longer than they need to be. This role will involve cross-site working across Bristol and Weston.

The EMM Improvement Practitioner will support UHBW to develop and sustain best practices relating to ward-based care improving inpatient flow and discharges. This will involve building key relationships within clinical and non-clinical teams across the Trust and using continuous improvement and project management skills.

Current EMM focuses include: use of discharge lounges, Proactive Board Rounds including SAFER principles, Criteria to Reside, timely and weekend discharges, 'Tap to Transfer' and Criteria Led Discharges.

Main duties of the job

  • Work with the EMM Senior Clinical Lead in embedding best practice in ward-based care, enabling effective patient care and flow
  • Attend and participate in multi-disciplinary board rounds/meetings to support the delivery of a co-ordinated multidisciplinary service.
  • Organise and prioritise time effectively and be able to meet deadlines and work effectively under pressure.
  • Use analytical skills to support decision making, develop conclusions and recommendations relating to flow and discharge.
  • Utilise continuous quality improvement methodologies to solve problems and develop intervention suggestions.
  • Use strong project management skills to ensure a methodical approach to plans agreed by the EMM programme team.
  • Prepare and monitor performance metrics for the EMM portfolio.
  • Interpret complex information and use this to develop sustainability and improvement plans to support inpatient flow and discharge.
  • Work with programme managers, and the Trust performance team in ensuring robust progress and highlight reports.
  • Support ward teams to embed EMM reviews into departmental and divisional meeting structures, using outcomes to develop ongoing improvement plans.
  • Work with the Trust Ward Accreditation team to ensure EMM processes are included within the accreditation process.

Person Specification

Knowledge and Experience

Essential

  • Leadership experience in a medium/large scale acute healthcare provider setting
  • Understanding of current NHS National Policies
  • Demonstrable understanding of quality and risk management issues and standards
  • Experience of monitoring outcomes and benefits of improvement projects
  • Experience of managing / supporting improvement projects during periods of change
  • Working with staff both within and across various disciplines


Desirable

  • Previous experience in using quality improvement tools and approaches in a healthcare setting
  • Experience of working with discharge and clinical site management teams
  • Experience in coaching or mentoring clinical and/or non clinical professionals in the application of improvement activity
  • Experience of using project management methodology
  • Experience of working with multidisciplinary teams and managers

Qualifications

Essential

  • Educated to masters level in a relevant subject (e.g. leadership / management) or hold an equivalent level professional qualification or demonstrate equivalent level of experience
  • Evidence of continued professional development


Desirable

  • Quality Improvement qualification (e.g. QISR, FLOW, QI Fellow, Lean)
  • Coaching or mentoring qualification
