This is an exciting opportunity for an enthusiastic, confident, well-motivated, autonomous RGN, who works well in a team, as well as on their own, to join our Acute Heart Failure service at Worcestershire Royal Hospital and the Alexander Hospital, Redditch, caring for patients with chronic heart failure in this rewarding area of work.

The successful applicant will be a registered first level nurse who ideally has experience of working with Heart Failure and cardiology patients but nurses with broad acute medical experience will be considered.

The successful candidate will be a good team player and be able to effectively manage their own time and workload. The successful candidate must be computer literate a car driver and willing to undertake training.

Main duties of the job

The post holder will provide specialist clinical skills and knowledge within this area of care and will assist to develop specialist education.

The post holder will act as the key worker and liaise with other healthcare professionals to provide the best possible specialist care for their patients. They will work in closely with staff across both WAHNT sites to facilitate the appropriate and efficient transfer of patients into the community with specialist and appropriate support.

To support the safe and effective running of an ambulatory diuretics service at both Worcestershire Royal Hospital and Alexandra Hospital Redditch

The role involves supporting the running of Rapid Access Heart Failure clinics alongside a team of Heart Failure Consultants and Senior Acute Heart Failure Nurse Specialist as well as providing an out-reach to medical and surgical wards across the trust to ensure that patients diagnosed with Heart Failure receive optimal evidence based care.

Job responsibilities

This post will support the development and implementation of a comprehensive and cohesive specialist service for clients and families of those diagnosed with the Heart Failure Syndrome. The post holder will act as a resource for members of the multi-disciplinary team supporting the clinical assessment and management of all Heart Failure patients and helping to ensure a comprehensive, patient centred, integrated service delivered by a multi-disciplinary team.

Develop, maintain and advance the practice and education of heath care professionals involved in patient care and act as a specialist to all personnel.

Have a key role in training and developing skills in other members of the Worcestershire Acute Hospitals Trust in the diagnosis, treatment and holistic approach for patients with Heart Failure.

Person Specification

Qualifications

Essential

  • First Level Registration.
  • Evidence of professional.
  • Development.
  • Evidence of management experience/training.
  • ILS.
  • Teaching and Assessing or Mentorship qualification.


Desirable

  • Degree or working towards.
  • Advanced Communication Course.
  • Qualification in Adult Holistic Health Assessment or willing to undertake.
  • Qualification in Non-Medical prescribing or willing to undertake.


Experience

Essential

  • Relevant experience at Senior Band 5.
  • Relevant experience caring for heart failure patients, preferably in a Cardiology setting.
  • Delivery of education particularly to peers.


Desirable

  • Caring for patients and their families in terminal situations.
  • Development and implementation of polices and education programmes.
  • Advanced life support.
  • Venepuncture
  • Cannulation
  • ECG interpretation


Skills / Knowledge

Essential

  • Drug therapies used in heart failure.
  • Knowledge of NICE & European guidelines for the management of acute and Chronic Heart Failure
  • IT literate.
  • Evidence of research based practice.
  • Organisational skills.
