Job summary
This is a new and exciting opportunity to embed the principles of the Community Mental health Framework within the Newton West Primary Care Networks.
The Assessment and Brief Intervention role is a new post that will promote positive working relationships between Voluntary, Community and Social Enterprise (VCSE), primary and secondary mental health providers. Clinicians who excel in this role tend to be passionate about clinical practice and have a desire to build positive working relationships with colleagues working across all agencies.
As an Assessment and Brief Intervention Clinician, you will have the opportunity to shape this new role working in collaboration with partner agencies. You will be well supported by the Teign Locality Mental Health Team and supported by peers and managers who are deeply invested in your success and are committed to your personal and professional needs.
Main duties of the job
Working as dedicated practitioner to the Newton West primary care network (PCN) as part of PCN multiagency team alongside mental health treatment practitioners, psychology, psychiatry and social care.
Support shared decision-making about self-management.
Facilitate onward access to treatment services.
Provide brief psychological interventions, where qualified to do so and where appropriate.

Main duties of the job
Working as dedicated practitioner to the Newton West primary care network (PCN) as part of PCN multiagency team alongside mental health treatment practitioners, psychology, psychiatry and social care.
- Provide a combined consultation, advice, triage and liaison function.
- Work with patients to:
Support shared decision-making about self-management.
Facilitate onward access to treatment services.
Provide brief psychological interventions, where qualified to do so and where appropriate.
- Work closely with other PCN-based roles to help address the potential range of biopsychosocial needs of patients with mental health problems. This will include the PCNs MAT, including, for example, PCN clinical pharmacists for medication reviews, and social prescribing link workers for access to community-based support.
- Operate without the need for formal referral from GPs, including accepting some direct bookings where appropriate, subject to agreement on volumes and the mechanism of access between the PCN and mental health team.

- Hold a Registered Mental Health Nurse, Occupational Therapist or Social worker Professional qualification
- Educated to Degree level or equivalent experience
- Diploma level specialist training or equivalent experience
- Evidence of continuing professional development
