Job summary

An opportunity has arisen within our established acute medicine department to appoint in to our new CESR Acute Medicine Posts. This post is designed to provide clinicians with the support and training required to successfully complete all aspects of the CESR process, gaining a CCT in Acute Medicine. There is a specifically designed training and teaching package, including secondments to ITU, HCE, respiratory and cardiology. All successful applicants will be assigned a Consultant Educational Supervisor.

Main duties of the job

The successful candidate will be post MRCP and have demonstrable experience of managing the acute medical take and ambulatory care. We are particularly seeking candidates with a strong background in medical education.

This is an excellent opportunity to work in Acute medical unit (AMU) and acute assessment unit at Derriford Hospital, Plymouth. We are a tertiary referral centre for various medical and surgical specialities, and we are complimented by tertiary trauma centre serving the South West population.

You will be joining a team of enthusiastic AMU consultants, registrars and junior team with aspirations of constantly improving our services and focussed to improve patient journey. We are a team of 10 Consultants, 5 Registrars, three Advanced nurse practitioners (ANP), approximately 10-12 Core trainees and 6-8 Foundation trainees. We are one of the most innovative Acute Medical Units in the South West. We aim to deliver unscheduled care to a high quality, within our Acute Medical Unit and Emergency Department. We aspire to constantly improve our services and focussed in exceling the patient journey. Our acute assessment unit (AAU) is well established with acute care pathways aiming to enhance our patient care and experience. It sits within the Acute Assessment unit which offers a specialised frailty pathway, primary care streaming and acute GP service. We are well supported by various medical and surgical speciality teams.

About us

AMU provides 51 assessment beds assessing 18,000 patients annually. Supervised participation in delivery of the Post Take ward round would be an integral part of this post. We also incorporate a rapidly expanding acute assessment unit which sees in excess of 60 patients a day. Registrar shifts are routinely rostered on AAU.

Our services are supported by an on-site Acute GP service providing triage and clinical input. An adjacent Short Stay Ward enables rapid review and turnover of acute medical problems.

60% of our medical take is through self-referral to ED. We work closely with this department and provide consultant cover with a dedicated Acute Physician in Charge (APIC) shift to stream and asses patients at the front door.

You will be managerially and professionally accountable to Dr Girish Bommayya, service line clinical director in acute medicine.

Job responsibilities

We are pleased to announce the opportunity to apply for the post of CESR Speciality Doctor at one of the most innovative Acute Medical Units in the South West. This role would be ideally suited to those following the CESR route for Specialist Registration. The role is well supported, with on-site consultant cover from 8am to 8pm 7 days a week.

The post holder will contribute to the on-call rota for medicine and provide sessions of direct clinical care within the Acute Medicine Directorate. Banding would arise from participation in the Medical Registrar on-call rota, currently a 1 in 12.

We anticipate the successful applicant will be an active member of the departmental team participating in identifying opportunities to improve our service and guiding these changes.

The post holder should expect to deliver unscheduled care to a high quality, within our Acute Medical Unit, Acute Assessment Unit and Emergency Department. The post holder will contribute to teaching rota (undergraduate and postgraduate) and clinical governance activities. The post holder is encouraged to develop a speciality interest within the scope of the department with also research related ideas, QIP and audits.

We anticipate that the CESR process will take between 3-4 years. Arrangements will be made for 3 month rotations through ITU, cardiology, HCE and respiratory during the post. Successful applicants will be expected to gain competencies in FAMUS, perform QIP activity at least annually and successfully pass the SCE in Acute Medicine. ALS certification must be, and remain, current throughout the post. There is a study budget and leave policy in line with training grade registrar posts.
