Job summary

This is a fixed term post for 11 months, due to funding.

If you are a Swansea Bay University Health Board employee this post will only be offered on a secondment basis and you will need to have gained your line managers approval for the secondment prior to going to interview.

Swansea Bay UHB Urgent Primary Care Service is offering an exciting opportunity for highly motivated and experienced Pharmacists to support the Urgent Primary Care (UPC) service. You will be responsible for improving the quality and effectiveness of medicines management in line with Prudent Healthcare principles and for developing, delivering and monitoring medicines management services within UPC. You will deliver pharmaceutical care to a range of patients including those who may be frail and vulnerable and have complex needs, via triage or face to face appointments.

You will provide expert pharmaceutical advice, education and training to prescribers and other healthcare staff working within UPC, developing close working relationships with a variety of health and social care professionals to maximise seamless care, ensuring the best outcomes for patients. You will develop and extend the role whilst ensuring scope of practice is in line with competencies as defined within individual personal boundaries of clinical practice.

Main duties of the job

Support the Urgent Primary Care (UPC) service to improve the quality and effectiveness of medicines management in line with Prudent Healthcare principles

Reduce inappropriate poly-pharmacy and wasteful prescribingResponsible for developing, delivering and monitoring medicines management services within UPC

Provide pharmaceutical care to a range of patients including those who may be frail and vulnerable and have complex needs, via triage or face to faceEngage patients in their care using a patient focussed approach and co-production principles

Utilise a holistic approach to support medicines optimisation in UPCProvide expert pharmaceutical advice, education and training to prescribers and other healthcare staff working within UPC

Develop close working relationships with a variety of health and social care professionals to maximise seamless care and ensure the best outcomes for patients

Develop and extend the role, ensuring scope of practice is in line with competencies as defined within individual personal boundaries of clinical practice

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
