CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!
This is an exciting, unique opportunity for an experienced Band 6 Occupational Therapist (or a Band 5 OT looking for a development role to Band 6) to join a dynamic, multidisciplinary community Cardiac Rehabilitation Team.
We are seeking the specialist skills of an enthusiastic, self-motivated Occupational Therapist who will work part time (0.8WTE) delivering care to patients following a cardiac event, cardiac procedure, or diagnosis of heart failure.
We're looking for a confident, independent practitioner with excellent communication skills.
Main duties of the job
You will work alongside physiotherapists, exercise therapists, nurses, dietitians and other OTs offering a combination of face-to-face and virtual rehabilitation, potentially working across 4 sites, but based in Hemel Hempstead.
A full valid driving licence and access to a car to use for business purposes is essential (unless you have a disability as defined by the Equality Act 2010). CLCH Hertfordshire Division covers 4 localities in West Hertfordshire, delivering care across the population.

- To provide specialist Occupation Therapy skills and knowledge as part of a multidisciplinary team to deliver a Community Cardiac Rehabilitation Programme for patients following a recent cardiac event, cardiac procedure or with a diagnosis of heart failure
- To take a lead in the provision of the Occupational Therapy interventions and delivering the emotional health and wellbeing component of the cardiac rehabilitation programme
- To provide highly specialised assessment and treatment for patients with complex needs, and to provide cardiac rehabilitation expert advice, guidance and information to patients, health and social care professionals, carers, relatives and other non-professionals
- To provide a specialised resource for other staff and healthcare professionals
