Job summary

This is an exciting opportunity to take on the role of team leader for the integrated discharge team based at the Manchester Royal Infirmary. The post holder, supported by senior staff will provided day to day leadership, clinical expertise and line management to the health staff working within the Integrated Discharge Team.

An adaptable, resilient and visible leader, you will have a person-centered approach and an analytical attitude to solving complex problems in order to inform decision making, whilst ensuring consideration is given to both the individual and wider variants.

Excellent communications skills are essential for liaising with patients, their families and colleagues, and you will have plenty of vision, supported by a forward-thinking attitude.

It is essential that you are a registered nurse or AHP and you will have sufficient experience at Band 6 AFC or above.

Main duties of the job

The post holder will deputise for the discharge service manager and provide day to day management of the health staff within the discharge team, they will also work as part of the wider team that supports hospital discharge; in collaboration with other services, including CHC, hospital staff and community teams to ensure the timely assessment and appropriate discharge of complex patients from the hospital.

Person Specification

Experience

Essential

  • Broad experience as a registered nurse or Allied Health Professional
  • Substantial post registration experience
  • In depth specialist knowledge of complex health and social requirements for discharge of patients and discharge to assess pathway.


Desirable

  • Exoerience in the application of the NHS Continuing health care screening and decision support tool
  • Experience in the application of the Mental Capacity Act with regard to discharge planning
