The South and West Hertfordshire Health Care Partnership (SWHHCP) are striving to reduce health inequalities and improve the health and wellbeing of its residents with a key focus on prevention and are delivering a number of key transformation project in order to implement an ambitious plan, as set out by the Hertfordshire and West Essex Integrated Care System.
We are extremely proud that the first Covid-19 virtual hospital in the country is in our portfolio and the success of this service has inspired the system. Heart Failure and COPD Virtual Hospitals are now operational and soon to launch are the Acute Respiratory Illness, and Frailty pathways.
The Virtual Hospital Performance and Delivery Lead will be critical in driving forward the continued success of the Virtual Hospital model. We are seeking a patient focused, ambitious, and forward thinking individual who can perform at a high level in a strategic environment. They will be driven and have the capability to lead the virtual hospital teamthe deliver the required level of performance and quality. on behalf of the SWHHCP.
Main duties of the job
Although this role is hosted by Central London Community Hospital Trust (CLCH), the successful candidate will work in partnership with clinicians and operational managers from West Herts Teaching Hospital Trust (WHTH), CLCH and system partners to ensure effective utilisation and delivery of all the virtual hospital pathways.
The Performance and Delivery Lead will have an expanding portfolio, taking ownership and leading on Virtual Ward development, prevention of admission and supported discharge initiatives under the SWHHCP transformation programme.The Virtual Hospital has already delivered significant bed day savings through reduced length of stay (LOS), reduced admissions and have improved prevention of admission.
