Job summary

The post holder will take the lead for the Cancer pathways within the Trust; ensuring that trust-wide data collection mechanisms are robust and that the Trust meets the Cancer Waiting Time targets.

Across the Trust, the post holder will promote consistent recording and monitoring techniques to ensure that all patients with cancer or the suspicion of cancer are included in reports and highlight all breaches and potential breaches to the care group management team.

Working with key individuals towards the further development of the cancer information system for the collection of data and monitoring purposes. This will include the production of monitoring reports whilst ensuring at all times that the information provided is of high quality and of clinical relevance.

Monitoring whether standards of care comply with the requirements set out in the NHS Cancer Plan and the Cancer Reform Strategy.The post holder will work with clinical teams across the trust to promote effective working practices to ensure that all patients with cancer or the suspicion of cancer are treated within target times.As manager of the Cancer MDTC Team, the post holder will supervise, the MDTC team and Cancer Pathway Tracking staff

Main duties of the job

The post holder will also collect data on a specific tumour group, and will work with the clinical team to reduce breaches of the Cancer Waiting Time targets. Working knowledge of Microsoft Office products (Excel, Word, Access) is required.

Excellent communication and negotiation/persuasion skills and the ability to work flexibly is essential.

The post holder will ensure that confidentiality as outlined in the Data Protection Act 1998 is adhered to at all times.
  • To have managerial responsibility for the MDT Co-ordinators and Cancer Pathway Tracking team.
  • Together with the Cancer Services Manger to have budgetary responsibility for the cancer admin team
  • To represent the Trust at local, network and regional meetings
  • To produce and submit cancer data monitoring reports in a timely manner
  • To produce and submit reports in a timely fashion when required
  • To be responsible for the tracking of patients on the Priority Target List ensuring that patients do not breach current and future National Cancer Waiting Times targets

Post Title: Senior Navigator & Pathway Manager

Department: Cancer Services, Division of Long Term Conditions & Cancer

Role of the Department:

The Cancer Administration Team is responsible for supporting the ongoing operational management and service development of cancer services, ensuring Service Level Agreements and National Access/ Waiting Time targets are met.

Duties are performed in partnership with specialist Multi-Disciplinary Team (MDTs), across the Trust, other NHS Trusts and the London Cancer Alliance.

The team is responsible for:
  • Ensuring that all suspected/cancer patients are appointed and tracked to meet the Cancer Waiting Times Targets.
  • Producing regular reports or providing raw data on delivery of activity against contract and National Targets.

Work force

The NHS Cancer Plan and the NHS Information for Health Strategy guarantees all patients will have access to an excellent quality of care and details the necessity for NHS Trusts and Cancer networks to improve the availability and quality of information for cancer patients. This is monitored by the two week wait, 31 and 62 day national Cancer Waiting Times targets.

The Cancer Reform Strategy published in 2007 outlines further the care of patients and the necessity of collecting data at Multi-Disciplinary Team meetings to support this.

All cancer information must be collected electronically at Multi-Disciplinary meetings from 2009.

This post is designed to act as the key support for the multi-disciplinary team in terms of ensuring timely and accurate collection of data relating to cancers producing reports and undertaking audit as required.

SCOPE & ROLE OF THE POSTHOLDER

Partnerships
  • Communications and Relationships
  • Provide and receive complex and sensitive information to both internal and external bodies regarding Cancer wait targets and all aspects of cancer data.
  • Use of negotiating/persuasive skills in providing feedback to both internal and external bodies regarding cancer waiting times requirements.

Internal:
  • To work closely with and report to the Cancer Management Team and Trust Cancer Steering Group.
  • To work closely with cancer lead clinicians, consultants, specialist nurses and other clinical staff
  • To work closely with the senior managers and ICT (Information and Communications Technology) Service Department
  • Liaise with patient administration when required.

External:
  • To represent the Trust at the London Cancer Alliance (LCA) information group for Data Managers and other similar forums
  • Liaise with the Thames Cancer Registry, the local Primary Care Trust and local Trusts.

Key Result Areas & Performance:

Data Management:
  • Implementation of complex data collection. Define data sets and design systems for the collection of cancer information. These may be paper based or computer systems.
  • To have the lead responsibility for ensuring the quality of information and to work with the Trust Cancer Strategy Group and Thames Cancer Registry to improve the quality of information and to ensure compliance with local and national reporting standards.
  • To prepare routine reports for Region, Primary Care Trust and the Trust. To prepare ad hoc reports when required and to present data at meetings.
  • To produce exception reports regarding validation within agreed deadlines.
  • To compile regular monitoring reports for the Division indicating progress towards agreed standards and highlighted problem areas.
  • To attend regular MDT meetings to gather information/data and provide support as required.
  • To participate in projects and provide data for service improvement as requested by members of the multi-disciplinary teams.
  • To support and work with clinical teams in completing clinical databases.
  • Contribute to the continuous development of information systems to improve data collection.
  • To provide feedback to clinical teams of those patients who need further investigations or appointments.
  • To ensure that all waiting times and clinical data is collected by the Cancer Data Team.
  • To ensure that cancer data is reported to the Trust and the Department of Health within appropriate timescales.
  • Implement policies and propose changes to practices, procedures for own area and impact on beyond own area
  • To maintain confidentially and actively encourage all staff to abide by this professional code
  • To highlight all breaches and escalate potential breaches to the care group and Trust Cancer Management Team.
  • To set-up and embed complex processes to improve the recording of cancer waiting times data.
  • Liaise with joint cancer centre to ensure that patients transferred from the Trust meet cancer waiting times.
  • To track all patients through the patient pathway.
  • To keep the Divisional Manager updated on developments in Cancer Waiting Times.
  • To promote an environment conducive to high client and staff morale
  • Ensure smooth operation of work environment
  • To ensure that all Trust policies and procedures are followed.

Line Management
  • To provide line management support and supervision to Cancer pathway trackers and MDT Co-ordinators
  • To ensure that data is collected and input in a timely and accurate manner.
  • To ensure objectives are set and that annual performance and development reviews and plans are made in conjunction with Trust policy.
  • Budgetary responsibility for the cancer data team

Educational
  • To maintain knowledge of local and national standards and ensure that information regarding new developments or changes to these standards, is effectively communicated to the Trust Cancer Strategy Group and to ensure appropriate action is taken.
  • To work with all multi-disciplinary teams covering patients to ensure that teams are aware of Cancer Waiting Times requirements.
  • To work with all multi-disciplinary teams covering cancer patients to improve the standard of recording information on the key milestones in the patients journey through diagnosis and treatment.
  • Evaluates the quality of own and others work and takes appropriate action.
  • Provide training to others within own specialist knowledge.
  • Work to achieve agreed objectives and is given freedom to do this working within broad professional or organisational polices.

Professional
  • Represent the Trust on the regional, network and local cancer information groups and be responsible for communication progress within the Trust to the groups and reporting from the groups information or action required.
  • Responsible for designing, developing and implementing effective systems for the routine collection and analysis of core cancer data across the Trust, ensuring liaison and collaboration with Cancer Lead Clinicians, Managers, Specialist Nurses and ICT (Information and Communications Technology) Service Depart
