This exciting role will appeal to dynamic and committed nurses or allied health professionals looking for that next step in their career. We are recruiting a Band 6 Frailty Practitioner, who will be Registered Adult Nurse, Therapist or Paramedic and have access to their own vehicle and be able to drive for work purposes.
The Frailty Support Team service within Southern Health is a unique model of care nationally. We support acutely unwell adults at home, who are experiencing a sudden decompensation in physical, psychological or social well being. Our primary aim is to provide first class health care, in the home, giving our service users the choice to remain at home and recover more quickly.
The Team is truly multi-professional, we currently have Consultant Practitioners, Physiotherapists, Occupational Therapists, Ambulance Frailty Responders, Paramedics, Nurses, GPs and Administrators. While we all work to our core strengths and abilities, we also help each other to develop new skills and acquire new knowledge that enables us to provide truly holistic patient centred care.

Main duties of the job
The Frailty Team aspires to provide a responsive, safe effective caring service that is well led. The team is commissioned to reflect the needs of the local community. Service aims include urgent case management, proactive case management and self-management and adoption of a co-produced approach with patients and service users. The frailty team, whilst operating from a local hub, delivers a joined up approach across a specific locality, to deliver excellence in care and engages with all practices, community teams and wider stakeholders in the locality.
