There are eight Primary Care Networks (PCN) in Ealing which are grouped into three localities. Each locality has a INT Leadership Team, which is composed of members of the community services, social care, primary care and ICB borough teams as well as relevant acute teams and voluntary & community sector organisations.

This role will lead the Integrated Neighbourhoods Programme in Ealing, including the development of strategy and planning for the programme, working closely with the Ealing Borough Based Partnership (BBP) Delivery teams.

While employed by the West London NHS Trust, this role will be deployed within the NWL ICS akin to a secondment. Line management will be carried out by the Trust, with day to day supervision and operational guidance provided by the Head of Primary Care - Ealing Borough Delivery Team NHS North West London.

This role will be on a fixed-term contract for 12 months.

Main duties of the job

Following the establishment of Integrated Neighbourhood Teams (INT) last year, this role has been created to lead the INT programme in Ealing, working with INT Leadership teams and the NWL ICB Ealing Primary Care Team to support and enable them to develop and mature to
  • Provide a holistic and fully integrated, holistic, timely and place-based care to the registered or resident population in the INT areas
  • Support the transformation agenda to enable all services and teams to function as a single team by breaking down historical barriers between organisation and teams
  • Mobilise and maintain effective and targeted multi-disciplinary team (MDT) meetings/forums organised around specific long-term conditions, social care needs, frequent use of emergency services, etc
  • Continue building and strengthening relationships within the INT.

This role will lead the Integrated Neighbourhoods Programme in Ealing, including the development of strategy and planning for the programme, working closely with the NWL ICB Ealing Borough Delivery teams. At the heart of this exciting new opportunity, is ensuring that the profile of the INT to partnership working in each network is innovative and addresses neighbourhood inequalities to accessing care.

Key functions include:
  • Lead the Integrated Neighbourhoods Programme in Ealing, including the development of strategy and planning for the programme.
  • Provide and oversee the development and delivery of high quality PMO function for all programmes and projects undertaken by the INTs
  • Support the locality leadership teams in the design, implementation and promotion of a whole systems approach to integrated care and the integrated neighbourhood model.
  • Support the locality leadership teams with the creation and implementation of their Development Plans.
  • Oversight, facilitation and monitoring of the delivery and implementation of locality leadership team priorities, ensuring they align with wider Borough Based Partnership priories.
  • Develop and maintain effective working relationships with Primary Care Networks, General Practices, acute and community physical and mental health teams, social care, VCSE sector and Population Health as well as wider public and private sector partners within neighbourhoods.
  • Support neighbourhood teams to understand the national agenda for integrated health and social care and their role within complex systems of health and social care in adopting new ways of providing services which focus on prevention and wider determinants of health. Lead and manage transformation / innovation programmes to implement these new ways of working from research to implementation.
  • Coordination of workstreams, identification of interdependencies, risks and issues, unblocker for issues identified to enable priorities to progress
  • Produce written and verbal reports, papers and presentations for the BBP Board and wider stakeholders as required.

  • Relevant experience of working within NHS environment or experience of operational management/ leadership within NHS provider setting
  • Experience in change management, working with stakeholders and in partnerships managing senior stakeholders
  • Experience in developing new and innovative models of care, from scratch, to address unmet needs
  • Experience of working in the NHS or outreach work, or undertaking local engagement work, or experience of working in or with the voluntary sector, working in community groups
  • Excellent communication and influencing skills with ability to adjust the style and content of messages to appeal to different people with diverse perspectives
  • Ability to carry out analysis, write well-structured and compelling reports for variety of audiences, negotiate with senior stakeholders on difficult and controversial issues, and present to large and influential group
  • Plan over short, medium and long -term timeframes, problem solve and respond to sudden unexpected demands
  • Prioritise own work effectively and be able to direct activities of others
  • Make decisions autonomously, when required, on difficult issues, working to tight and often changing timescales
  • Identify and interpret National policy and undertaking research in order to reach a well-rounded position.
