An exciting opportunity has come up for an Specialist Clinical Practitioner to work within the Baywide community health team supporting the homeless population within Torbay on a fixed term contract until 31st March 2024.
The post holder will work closely with all existing homeless services in Torbay to provide wrap around, person centred trauma informed responses to this cohort of people in line with recently implemented Community mental heath service framework.
Please note interviews will be taking place on Monday 19th June 2023.
Main duties of the job
The post holder will assess individuals without referral from colleagues offering assessment of complex individuals, outlining management plans, delivering interventions independently where necessary, and coordinating broader psychosocial interventions. They will also support other practitioners where required. The post holder willwork with people in Torbay who have a mental illness, are experiencing homelessness ,rough sleeping, living in hostels and temporary accommodation or sofa-surfing, and are also suffering the effects of one or more of the following domains of deep social exclusion:
The post holder will provide skilled engagement through relational expertise, assessment/formulation and specialist input to case management.

Qualifications
Essential
- Clinical based Degree. Qualified as RMN.
- Minimum of 60 credits at post-graduate level in relevant subject (e.g. Advanced psychosocial assessment and formulation, Enhancing the Care and Support of the Person with Dementia and their Carer) or equivalent experience.
- Evidence of Continuous Professional Development
- Current registration with the NMC with no restrictions.

- Post basic qualification in speciality Application Non-Medical Prescriber (V300)
