The Adult Southampton Service line (ADS) provide community specialist services, community neuro services, and an early supported discharge provision through Snowdon at Home. We're looking for a Service Manager for Community Neuro and Specialist Services who will play a pivotal role in providing leadership and operational direction to the department.
As Operational Manager, you will be responsible for ensuring effective systems are in place to address data quality issues and ensure effective delivery and to lead the operational embedding of the integrated models of care within the team including shared practice, shared teaching of patient caseloads.
Main duties of the job
*To work with the team to develop and maintain the capacity modelling and escalation process to ensure the effective operational delivery of services locally.
*To lead the development and delivery of streamlined pathways of care associated with the service delivery of the Community Neuro and Specialist Services.
*To lead the development of the service in terms of operational aspects working with the Community Neuro and Specialist Services Clinical Matron/Lead to support the improved quality/quantity of patient care.
*To work proactively and collaboratively with multiple service stakeholder including sister Solent services, secondary care, primary care and the voluntary sector to support streamlined pathways for patient care.
*To take the lead for the development and delivery of specific workstreams identified by the Services Manager and to support implementation of additional workstreams across the URS.
