You will be joining KMPT at a highly exciting time. The work around the implementation of the Community Health Framework for mental health within the PCS will enable Social Work to develop a strong profile within this sector. As a Senior social worker, you will provide a high standard of support and care delivery with regards to social work needs to those service users whose ability to cope with the tasks of living are threatened or impaired by mental health issues, emotional, psychological and social problems. You will work closely with other agencies and individuals to support service users with a clear focus on recovery, social inclusion and social justice.

Main duties of the job

The role will entail assessment, support planning and the opportunity to provide 1-1 strength based therapeutic interventions. You will act as Care-Coordinator, (where applicable) with individuals who have serious and enduring mental health difficulties. You will be a core member of the leadership team within Dartford CMHT providing expertise and clinical support to the team.

Job Summary
  • The post holder will be working within a MPT covering a defined geographical area of the Trust.
  • Within the CMHT Social workers will focus specifically upon the needs of working age adults with mental health issues.
  • The post holder will be registered with Social Work England
  • Deliver social work practice which is informed by statutory guidance, best evidence, and research knowledge.
  • Provide professional guidance, support and developmental opportunities with less experienced or knowledgeable social work colleagues, including taking the role of supervisor, practice educator with social work students.

Core Responsibilities
  • To develop and deliver evidenced based psychosocial interventions where appropriate. This will include the Change Model for individuals diagnosed with Complex Psychological Issues (Personality Disorders) and undertake training where appropriate.
  • To undertake holistic social work assessments using strengths based model collaboratively with services users, relatives and carers in order to promote the appropriate support towards recovery.
  • To undertake a range of procedures and practices underpinned by theoretical knowledge or relevant practice.
  • To provide a service that is tailored to meet the needs of the individual and to understand what our service users need and be adaptable and responsive.
  • To ensure effective communication with particular regard for individuals who may have difficulties in relating to the complexity of emotional and psychological conditions. This will include skills in managing moods, motivation, counselling, training, empathising and reassurance.
  • To manage an appropriate complex caseload requiring social work interventions from a senior social worker
  • To work with a degree of independence without the need for close supervision, whilst recognising when and how to seek advice from a range of sources. Use supervision to identify strategies to build professional resilience and balance the potential for bias in decision-making.
  • To provide and implement professional supervision processes to more junior social work staff, to support evidence-informed judgements in complex cases.
  • To ensure appropriate risk assessments and risk management plans are completed and reviewed.
  • To ensure all child protection, adult safeguarding, Mental Capacity Act issues are dealt with according to policy and procedure.
  • To undertake the function of Care-Coordinator in line with Trust policy for the Care Programme Approach where appropriate.
  • To work collaboratively and in partnership with service users, relatives, carers and others in order to seek community based solutions.
  • Communicate with relatives/carers in accordance with local protocols ensuring the plan of care/aftercare needs is communicated effectively.
  • To refer to the Local Authority in respect of assessments under the Care Act where required.
  • To work collaboratively in partnership with other team members, other agencies both statutory and non-statutory.
  • To liaise and develop close working relationships with Primary Care Agencies.
  • To make appropriate referrals to statutory agencies and participate in multi-agency working such as MAPPA, MARAC where appropriate.
  • Act in such a way as to uphold the rights of the service users and support individual rights to self-determination and confidentiality.
  • To positively address issues of discrimination and social exclusion.
  • To be professionally curious and creative.
  • Support service delivery and improvement by embracing challenges and seeking constructive ways to make positive changes.
  • To evaluate and reflect upon the quality of support given and the goals achieved.

  • To ensure you undertake and maintain your CPD in line with the annual registration process with SWE.

Leadership and Staff Management Responsibility:
  • To be professionally accountable for own activities and duties.
  • To support the learning of others to be Peps1 and 2 trained.
  • To provide professional supervision to band 6 Social Workers
  • To take a lead role within the multi-professional context in enabling service users and carers to exercise their rights and promote their equality and diversity.
  • To undertake a leadership role in respect of practice and service development .
