Sussex Partnership have a brand-new opportunity for a Mental Health Liaison Practitioner to join our friendly and established Community Mental Health Assessment and Treatment Service based in the beautiful town of Horsham.
We work with adults living with mental illness, supporting their recovery and the development of skills and strategies to support them to maintain their wellbeing. We are a lively and friendly team where you will be offered plenty of support along with mentorship and supervision from seniornursing colleagues, active management support and the chance to undertake training courses relevant to your professional practice.
This post offers The Trust's Golden Hello for Band 5 or 6 Nurses, or a Relocation package up to a maximum of �2,000. (Pro rata applies and subject to terms and conditions. Please see attached Protocol document)
Main duties of the job
As a Mental Health Liaison Practitioner in our service you will be working as part of a multidisciplinary team which includes Psychologists, Nurses, Occupational Therapists and Medics to develop care plans in partnership with our service users.
Your role will be to work as part of the secondary care mental health service, acting as a link clinician to Primary Care Network teams. You will need to undertake routine assessments which have been triaged to the role.You will need to be able to maintain close working relationships with local secondary care teams, keeping them informed of developments within primary care.
As well as be able to build professional relationships within Primary Care Networks to support effective development of care pathways within localities

- Current registration as a Registered Nurse, Social Worker or Occupational Therapist with experience working in mental health
- 3 years post-qualifying experience of clinical work with exposure to a range of mental health problems
- A team player who is willing to collaborate and strive for best practice
- Experience in conducting clinical assessments and risk assessment
It would also be advantageous if you have:
- Experience within a community assessment and treatment service
