An exciting NEW opportunity for a specialist in VTE Prevention and Thromboprophylaxis has arisen within our team at The Royal Berkshire Foundation Trust
Are you looking for a new challenge? Do you want a role that involves patient interaction, alongside teaching, audits and service evaluation and quality?
We are looking for an enthusiastic and motivated pharmacist to join our Clinical Pharmacy team to lead, develop and promote best practice within thromboprophylaxis and VTE prevention.The post holder will act as an expert clinical practitioner developing and best practice in VTE prevention. You will work closely with the Trust Thrombosis lead and patient safety teams but also will be independent and autonomous with its own daily workload.
You must have hospital pharmacy experience involving clinical pharmacy skills as well teaching skills. This role requires the ability to work effectively as part of a team, through excellent communication and interpersonal skills as well as the ability to motivate yourself and others to achieve common goals. To be successful in this application you will demonstrate confidence as a senior clinical pharmacist and be a role model to staff.

Main duties of the job
- Provide a specialist clinical pharmacy service to designated wards, clinics and specialist areas, with a particular focus on medicines within anticoagulation and VTE.
- Lead the safe, rational and cost-effective prescribing of medicines across key specialty.
- To participate in various consultant ward rounds and clinics within specialties identified as key areas
- Continued implementation of NICE guidance in reducing the risk of VTE across the Trust
- Monitor electronic VTE risk assessments and deliver engagement and surveillance in high admission areas optimising risk assessment for all patients.
- To audit VTE risk assessment numbers on a monthly basis and feedback to Patient Safety Group.
- Patients who develop a hospital acquired VTE, the post holder will work within multidisciplinary teams to complete root cause analysis investigations, reporting back to stakeholders all outcomes.
- To support anticoagulation, undertaken VTE assessments and support the MDT in decision making
- Provide Trust-wide extensive specialist support in an appropriate defined clinical area of care relevant to the care group. This will include developing collaborative partnerships across organisational boundaries.

- Masters Degree in Pharmacy or equivalent (from an accredited University registered with GPhC)
- Current registered practising Member of GPhC
- Postgraduate Diploma in Clinical Pharmacy, MSc or other Clinical Postgraduate qualification
