Job summary
West London Forensic Service has been chosen by NHS England to pioneer a new approach to support the discharge of Service users in Learning Disability and Autism medium and low secure forensic mental health settings and support their transition and community care. In the past a lot of people with learning disabilities have been placed in hospital units a long way away from their home, family and friends. The government has said this must end and people should get support and treatment close to home. This can improve quality of life, reduce inequality, and give hope to people who may have been detained for many years. It also saves significant expenditure on hospital care, which can be reinvested into community services. We are looking for a Forensic Band 6 CPN to be part of this innovative team.
The post will be based in Ealing, St Bernard's Hospital Site.
Main duties of the job
This role is a rare chance to be part of shaping a new service, providing opportunities to develop your skills and your career.
The post holder will need to have an understanding of psychosocial interventions to include psycho-education, wellness recovery action plans and discharge planning. The post holder will also be responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care for a defined caseload; and maintaining associated records.
The post holder will support the team manager in specifically delivering on the following key areas:

- To administer medication within NMC guidelines and the Trusts Medicines Code.
- To work with inpatient and outpatient clinical teams in identifying care needs and systemic risks. This may require liaison with external agencies responsible for the provision of resources, services, and vulnerable adult or child protection.
- To refer and consult with the home and host MAPPP authorities and to maintain accurate data collection in relation to this.
- To refer and consult with other public protection agencies as necessary, such as: - MARAC; JIGSAW; Channel Panel and other police services.
- To participate in completion of the HCR20 and appropriate risk assessments for individual service users.
- To work with Secondary and Primary Mental Health Services.
- To develop individual recall plans for forensic service users and ensure these are shared with the relevant parties.

- The applicant must have a minimum of two years post graduate experience
- The applicant must have two years post qualification experience in a mental health/LDA setting

- Demonstrate an ability to undertake assessments of risk using specific risk assessment tools.
- Demonstrate an ability to formulate implement and evaluate programmes of care for those individuals in the Community Forensic Service and those being referred
- Demonstrate knowledge of Part 3 of the Mental Health Act and its application to Mentally Disordered Offenders.
- Demonstrate knowledge of the Criminal Justice Act and its implication for the management of Forensic clients.
