Job summary

An exciting opportunity has arisen for an experienced Nurse, Occupational Therapist or Physiotherapist to take the lead role for dementia and delirium care at Calderdale and Huddersfield Acute Foundation Trust.

We are looking for a dynamic, innovative expert health care practitioner who can assume a leadership role to promote and support high quality care for patients living with dementia, delirium or related cognitive impairment within our busy acute and community localities. This is a high profile and exciting position, which will support the care of this vulnerable group of patients.

The post holder will need to have sound clinical skills in relation to dementia and delirium care in addition to a sound knowledge of relevant national and local standards. The most important qualities link to having a passion for high quality dementia care, excellent communication skills and the persistence to overcome challenges that can be translated into sustained service improvement.

The post holder will work as part of the senior team to ensure national and local standards are fully integrated into practice. The post holder will work with the full multidisciplinary team in addition to patients and carers to ensure a positive patient experience.

Main duties of the job

Lead on the sustainability of the West Yorkshire and Harrogate Dementia Partnership \Standards for Improving care for people with dementia whilst in hospital, and to actively participate in Trust achievement of other key national and regional objectives relating to Dementia Care such as the National Audit of Dementia, the National Dementia CQUIN, the NHS Operating Framework and NICE Quality Standards.

Work alongside and integrate into divisions to prevent avoidable admissions of patients living with dementia or high risk of delirium. Ensuring these people are cared for in the right environment.

Be an integral part of the Trust's senior structure providing strategic leadership regarding dementia, delirium & falls care.

Lead on patient experience and seamless quality of care across dementia services ensuring the cross divisional nature to patient pathways is not detrimental to care.

Be responsible for the effective development of dementia and delirium care in collaboration with the Allied Health Professions (AHPs), ensuring they are responsive to patient / carer needs across all MDTs.

Work with divisional leads to support local implementation of local and national falls care guidelines and policy.

Work closely with the Lead Clinician to ensure new developments, projects, targets, and objectives are met. Ensure dementia is incorporated within healthcare governance strategies.

Qualifications and Training

Essential

  • Previous experience of formal/informal teaching of patients and staff,
  • Registered general nurse or registered Allied Health Care Professional
  • Teaching/Assessment, Mentoring qualification
  • Masters' qualification or working towards - Preferably related to dementia care


Desirable

  • Non-Medical Prescriber


KNOWLEDGE, EXPERIENCE & EXPERTISE

Essential

  • Experience in working with people living with dementia and delirium
  • Experience in elderly care and frailty
  • Experience in working as an autonomous practitioner within a specialty
  • Working knowledge and clinical management of patients specific to role
  • Experience of undertaking quality improvement project
  • Knowledge of the key professional issues and NMC guidelines related to professional practice OR AHP guidelines related to professional practice
  • Experience of applying research evidence to clinical practice.
  • Evidence of continuing professional development.
  • Commitment to maintain own fitness for practice through maintaining a personal profile of competencies (profile)
