Job summary

Main duties of the job

o You will visit a number of patients on a daily basis in their own homes to provide a comprehensive range of clinical nursing activities.

o You will provide high quality nursing care to a range of patients with varying healthcare needs.

o Provide a high standard of holistic assessment and care planning.

o Administer IV drugs, complex wound management, health promotion and medicines management, end of life care and symptom control.

o Full time (37.5 hours per week) and part time opportunities available.

o working in Wokingham localities and across outlying areas.

  • Registered General Nurse (RGN)
  • NMC Registration


Continuous Professional Development

  • Evidence of on-going professional development


Desirable

  • Mentorship training


Previous Experience

Essential

  • Experience of undertaking general assessment by utilising evidence based care.
  • Community nursing Experience


Knowledge, Skills & Abilities

Essential

  • Well-developed written and verbal communication skills
  • Computer literate


Desirable

  • Ability to demonstrate an understanding of LTC's and the concept of self-management


Additional Requirements

Essential

  • To work bank holidays/weekends or unsocial hours to meet the needs of the service
  • Full UK Driving Licence and use of car for work purposes
