An opportunity has arisen in the NHS@Home team, based at University Hospitals Bristol and Weston General Hospitals. With growing evidence that supports the benefits of a 'home first' approach for our patients, there has never been a better time to be part of the expanding NHS@Home service.

We are looking for B5 healthcare professionals to join our small but friendly multidisciplinary team of nurses, pharmacists, physician associates, AHP and microbiology consultants. The NHS@Home service provides a 7-day service, currently 0800-1830 (no nights) but we can offer flexible working arrangements.

NHS@Home acts as an extension to our UHBW Hospitals in Bristol and Weston, enabling patients to go home to continue their hospital treatments in the comfort of their own homes. The B5 role in our team will allow you to continue to utilise and develop your acute clinical skills whilst applying them to the care of patients in their home setting.

As nurse in the NHS@Home team, you will be involved in community-based care, currently Outpatient Parenteral Antimicrobial Therapies, frailty, Heart Failure, Respiratory and general medical and surgical virtual wards.

Please note: Full time or part time working patterns, weekend work but no nights.

Main duties of the job

You will work autonomously, delivering individualised treatments and care to our patients in their own homes enabling our patients to leave hospital sooner and prevent hospital admissions

The post holders will be based out of the Bristol Royal Infirmary/Weston General Hospital, working as part of the Multidisciplinary Team within University Hospitals Bristol and Weston to provide specialist nursing care, assessment, education, and support to patients who are referred into the at home pathways.

Staff work daily in the community visiting patients in their homes and the post holder will be expected to hold a valid driving license and be a car owner with a valid insurance certificate covering business use as you will be required to use your own vehicle for daily visits. Your travel expenses will be reimbursed as per Trust guidelines, and we will provide residential parking permits.

Qualifications and Training

Essential

  • RGN.1st Level or other registered healthcare qualification
  • Current NMC/ registered healthcare Registration


Desirable

  • Training Qualification or equivalent experience
  • Relevant post registration course


Knowledge and Experience

Essential

  • Experience of caring for sick patients in a variety of settings
  • Current drivers license and access to a car daily - E


Desirable

  • Previous experience of speciality
  • Experience of taking charge


Skills and Abilities

Essential

  • Good basic level of English language demonstrated through effective written and verbal communication skills
  • Ability to solve problems and use initiative

  • Competency in medication administration including IV drug administration or willingness to undergo training
