This is an opportunity to join a well established community rehabilitation team.

The community service manages patients who may have complex neurological conditions such as Motor Neurone Disease and Stroke (including patients on the Stroke Early Supported Discharge pathway), acquired brain injury, long term neuro-degenerative conditions, patients with acute and chronic musculoskeletal problems, fallers, respiratory disorders, malnutrition and those at risk of malnutrition, and complex multi pathology conditions which may include patients with dementia or learning disabilities.

Main duties of the job

Clinical Duties
  • To provide a clinical dietetic service to patients in their own home and care homes who require nutrition support, and other types of dietary interventions as required.
  • To lead on the home enteral nutrition caseload.
  • To work autonomously within Trust policies and professional practice, maintaining accurate and comprehensive up to date records.
  • Take part in the education and training of Student Dietitians other Professional Students and non-registered staff working in the Multi-disciplinary team.
  • To deliver talks / presentations and group education sessions to both patients and Health care professionals as deemed appropriate.
  • To Support the Clinical Lead Dietitian in the day to day running of the Dietetic service in the community setting.
  • To contribute to the dietetic training and supervision of Assistant Practitioners, Dietetic Assistants and Rehabilitation Assistants.
  • The post holder will work as part of the wider Health and Social Care Team linking with staff from the Acute Trust, Primary and Social Care.
  • The post holder will be required to undertake lone working in line with Trust policy.
  • Administrative duties and general patient care across departments are required.
  • The post holder will be expected to demonstrate self-directed development and practice.

Job responsibilities

Professional Duties

  • To comply with and, along with the team leader, contribute to the development and implementation of protocols, procedures and guidelines both clinical and service specific. To give feedback on policies when required which may impact on the management and care of patients.
  • To participate in governance initiatives on a regular basis, measuring and evaluating individual and the teams clinical practice, e.g. CPD, clinical effectiveness, research and development, clinical audit and service activity data.
  • To ensure specialised treatments offered to patients / clients are based on the best available clinical evidence by keeping up to date with current literature, implementing research tools and liaising with other practitioners, both within and outside the service, in order to develop and maintain best practice.
  • To be responsible for the issue and safe use of equipment used in your area of work and to adhere to the appropriate service policies.
  • Along with the Clinical Lead / Advanced Practitioner generate and contribute to the collection, analysis and dissemination of data as required for the service, both manually and computerised.
  • To lead, develop and actively undertake clinical audit as requested.
  • On a day to day basis, to support members of the MDT and contribute to successfully meeting their objectives and development needs.
  • To assist the Clinical Lead to manage the performance of the team by having a delegated role in supervision/PDR.
  • To organise and participate in in-service training programmes, under direction, ensuring appropriate training is delivered.
  • To undertake appropriate development opportunities as identified at yearly personal development reviews and through personal development plans. To incorporate acquired knowledge into working practice as appropriate, following discussion with Clinical Lead / Team Leader. To disseminate information from training effectively throughout the team, the service and MDT as appropriate
  • To participate in the community division supervision process and peer review and maintain an up to date CPD portfolio including evidence of experiential learning, reflective practice and any relevant training or experience. This should demonstrate responsibility for maintaining own competency to practice.
  • To participate in the Trust Personal Development Review process taking responsibility to comply with an agreed personal development plan.
  • To give specialist advice, teaching and training to patients / clients, carers, volunteers and other health, educational and social care professionals, as appropriate to ensure provision of good patient care.
  • To be responsible for the supervision and education of student dieticians to graduate level in the role of clinical educator.
  • To contribute to the supervision and education of students from other professional groups e.g., Nurses, Physiotherapists, Speech and Language Therapists in as required.

Qualifications

Essential

  • Relevant Degree
  • Registered with HCPC
  • Clinical Educator Qualification


Desirable

  • Evidence of postgraduate training
  • Member of professional body
  • Attendance at relevant Special Interest Groups / Study events
  • PENG Course


Knowledge

Essential

  • Recent experience with adults demonstrating ability to assess, treat and evaluate dietetic care in acute or community setting
  • Good Clinical Reasoning skills
  • Evidence of CPD, self-directed learning and Reflective Practice
  • Awareness of relevant Legislation, NSF'S/ Local and National initiatives
  • Critical Appraisal Skills
  • Ability to maintain legible and accurate records
  • Understanding of Clinical Governance
  • Teaching/Presentation skills
  • Experience developing others, including student training.


Desirable

  • Previous experience in Community setting, complex neuro, home enteral nutrition
  • Experience in contributing to service improvements e.g. Audit, patient information etc.
  • Experience in dealing with complaints (e.g. diffusing complaint or investigating complaint)
