Main duties of the job

It is widely known that the majority of patients recover better at home, both physically and mentally.

Acknowledging this, Manx Care is working hard to change the balance of care from hospital to the community and streamline the discharge of patients from hospital as soon as is appropriate.

We aim to develop a Hospital Social Work Team that works alongside ward based Multidisciplinary Teams, as well as our Complex Discharge and Access and Capacity Teams, to streamline transfer of care and discharge pathways. This integrated team will lead discharge planning for those patients with complex needs who require coordinated care and support to return home.

As an experienced Social Worker, you'll be responsible for ensuring that patients who are being discharged from the hospital are able to access the services and resources they need to successfully transition back to their homes and communities. You'll work closely with patients, their families, and other healthcare providers to assess their needs and develop a plan for their ongoing care. This can involve providing emotional support, assessing patients' needs and abilities, and coordinating with healthcare professionals to develop a comprehensive discharge plan that addresses the patient's medical, social, and financial needs.

Person Specification

Qualifications

Essential

  • Social Work Professional Qualification (e.g. BA/MA Post Graduate Diploma in Social Work, Dip. SW, CQSW).
  • Be registered with a UK Social Work Regulator.


Desirable

  • Awareness of current IOM legislation.


Experience

Essential

  • Experience of working with older people and demonstrable knowledge of the psycho-social and physical process that underlie many experiences and challenges in later life.
  • Experience of completing assessments, including risk assessments and care planning.
  • Knowledge and experience of working with Safeguarding Issues.


Desirable

  • Experience of working in a hospital environment and an awareness of the challenges this can present.
  • Two years post qualifying experience as a social worker.
