Main duties of the job

As part of our Community Transformation Programme and exciting opportunity has arisen for a band 6 In Reach to Care Homes Worker to support the further development of our In reach to Care Homes Service. These posts will form part of our Enhanced Community Mental Health Team for Older Adults and will work alongside other Multi-Disciplinary Health professionals in delivering care to patients in the care home.

The aim of this service is to ensure that care home patients have an improved experience through high quality care and access to health professionals with the skills to support their Mental Health needs

We are looking for dynamic, highly skilled and motivated nurses with knowledge and experience in working with Older Adults.

You must be a registered mental health nurse,

As part of the Enhanced Community Mental Health Team, you will have access to regular clinical supervision and training and will be further supported in developing new skills required for this unique role

You must be able to independently meet the travel requirements of the role.
