Community Senior Registered Nurse

Main duties of the job
To manage a designated caseload of patients within the district nurse team. Is professionally accountable for clinical decision making regarding those patients on the designated caseload/s. To provide clinical leadership to the team, supporting both the District Nurse Team Leader and Junior Team Members. To manage the DN team and deputise in the absence of the District Nurse Team Leader. To undertake comprehensive assessments of the physical, psychological and social care needs of patients, including needs of carers. To provide individualised and person centred care plans that meet the persons health needs and that will facilitate care at home and prevent hospital admissions. To deliver safe, high quality nursing care to patients within their own home that is based on principles of best practice and current clinical evidence. To act as the patients advocate and facilitate the patients own choices with regards to nursing care, promoting independence and self-care where appropriate. To work autonomously, delivering care as a lone worker, using own initiative and with the responsibility to review care at each visit. To work closely with medical and paramedical personnel and other nursing service e.g. health visitors, hospital nurses, liaison nurses, hospice nurses, community psychiatric nurses, Long Term Condition Co-Ordinators and specialist nurses. To undertake the assessment of the immediate needs of new referrals received into the team.

About the role and our team

In this role, you will be delivering skilled and evidence-based care to patients who can often have highly complex needs. You will be working closely with the multi-disciplinary team to provide and facilitate care to patients and their families, working to support people in their own homes to meet their individual needs and avoid hospital admissions.
As a Senior Registered Nurse, you will play an integral role in leading, supporting and developing staff within your team in preparation for the changes ahead.
A key responsibility of this role is the management of a delegated caseload of patients, providing care in a range of settings including patients’ homes, residential homes and nurse-led clinics. You will also have the opportunity to manage the team in the absence of the Lead Nurse.
