The Pathway 3 team in West Kent are looking to recruit a dynamic,enthusiastic and adaptable Band 6 Occupational Therapist.

Pathway 3 is the Discharge to Assess service, we support transitionfrom acute and community hospitals to care homes or own homefor a period of assessment to determine long term need.

We work collaboratively with Kent County Council Social ServicesTeam, All Age Continuing Care (CHC) to complete the assessments in a timely manner, this assessment is for both health and social care needs which may include a financial assessment.

The team is multidisciplinary and the ability to work collaborativelyand proactively with our partner organisations and provider servicesis essential. You will be working alongside colleagues who are passionate aboutproviding high quality care for patients admitted to the pathway.You will also act as an advisor, supporter and educator to ourprovider services to ensure timely assessment of individuals on thepathway.

Main duties of the job

As a Band 6 Occupational Therapist you will work as part of the team to keep patients safe and well in their own homes and care homes these may be under the D2A process or our Non-Weight bearing scheme wherepatients are supported whilst recovering from fractures under thecare of orthopaedic teams.

You will use your clinical judgement to effectively complete functional assessment that support evidenced-based care planning, and to support discharge home

  • Evidence of Specialist qualifications relevant to area of practice: Care of the older person, Frailty, Long term conditions, Rehabilitation
  • HCPC registration minimum of 3 years
