Main duties of the job

The post holder will have the opportunity to gain experience rotatingthrough all areas of the pharmacy and clinical services, and will also contribute to the weekend and emergency duty service.

This will involve medication history taking and medicines reconciliation, clinical review of prescription charts, advice to patients and members of the MDT and clinically screening discharge prescriptions.

We provide excellent learning opportunities for staff with group clinical supervision sessions, supervision sessions with directorate leads and regular teaching sessions covering a range of topics.

Staff are further supported and encouraged to enrol in post-graduate studies such as clinical pharmacy diploma and pharmacist independent prescribing.

Job description

Job responsibilities

  • To provide clinical pharmacy input and maintain high standards of medicines management
  • To carry out medication histories, medicines reconciliation and clinical chart checks for patients admitted to hospital
  • To clinically and accuracy check discharge prescriptions, ensuring patients receive counselling and the opportunity for a discharge medicines service referral to improve transfer of care
  • To provide advice and support to patients to ensure that they get the best from their medicines
  • As part of the team you will work closely alongside other pharmacy colleagues, taking part within clinical supervision sessions and teaching
  • You will provide rotational clinical support to the directorates of emergency care, medicine, care of the elderly, surgery, aseptics and dispensary.
  • Work to deliver the 7 day clinical Pharmacy service by supporting the late night, on call and weekend rota


Person Specification

Qualifications

Essential

  • Master's Degree in Pharmacy
  • Registered Pharmacist with the UK General Pharmaceutical Council or currently completing Preregistration Pharmacist Training


Skills

Essential

  • Ability to communicate effectively both written & verbally
  • I.T. Literate
  • Evidence of CPD
  • Experience working within the hospital, community or general practice setting


Desirable

  • Diploma in clinical pharmacy or desire to work towards
  • Experience within the hospital setting
  • Specialist knowledge of pharmacy systems
