We are looking for a dynamic, innovative Specialist Doctor in Acute Medicine to join our team! This is a brand-new role, created following the SAS contract reform in 2021. The post holder will need to demonstrate the generic capabilities laid out in the framework for the new Specialist grade, to the level equivalent of a Consultant in Acute Medicine. The post-holder will also be actively participating in medical student and junior doctor teaching.

We are expanding our urgent and emergency care workforce - emergency medicine consultants and nurses, acute physicians and advanced care practitioners. Our Acute Medicine Department is based at Stoke Mandeville Hospital in Aylesbury, Buckinghamshire, and consists of the Acute Medical Unit (AMU) and Ambulatory Emergency Care Unit (AECU) within Same Day Emergency Care (SDEC).

Full and part time hours will be considered.

The successful applicants will join a workforce of Consultants.

The post holder will participate fully in the provision of Acute Medicine including clinical governance and the delivery of both Trust and Department of Health Clinical Quality Indicators. The post will be based across the Acute Medicine Department and medical take at Stoke Mandeville Hospital.

Job description

Job responsibilities

This post has responsibility for patients managed under the Acute Care Hub in each of its three areas, supported by a junior doctor team. The Acute Care Physician (ACP) consultants run the Hub out of hours in conjunction with the speciality physicians (SCP).

The Acute Medical Unit (AMU) and parts of Same day Emergency Care (SDEC) Unit are the area for assessing patients by the medical team during the medical take. The medical on-call receives about 35 patients with decision to admit in 24 hours. It is supported by the consultant Physician of the Day (POD) made up from a combination of ACP and SCP. Junior doctors rotate through the medical on calls and there are a day shift of 1x FY1, 1x FY2+ (FY2, IMT or GPVTS) and 1x ST3+ all day with additional FY2+ and ST3+ on a late shift. The AMU is run Mon-Fri by the acute medicine senior and junior team. Overnight the AMU is mainly run by a FY2+ and ST3+ with an FY1 and IMT/GPVTS covering the wards (supported as needed).

The Ambulatory Emergency Care Unit (AEC) was opened in November 2014 and has nursing cover 08.00-20.30 with an ACP consultant and junior doctor covering 09.00-17.00. IMTs are extending doctor cover until 8 pm weekdays and 9-17:00 weekends. This includes running a virtual ward and some medical day case patients. As we recruit more consultants this unit will have increased consultant cover.

At the weekend we have a front door team covering medical admissions and AMU, a SSW team, a respiratory team, a gastroenterology team and a ward cover team. The weekend ward cover team which has a list of patients admitted Fri\Sat\Sun to ensure any deterioration in these patients is detected over the weekend where the speciality teams are not present. This post will cover weekend duties covering the front door with AMU, SSW and wards in a 1:8 rotation.

For bank holidays (Monday) including Easter (Friday & Monday) the Acute Care Physicians provide cover for the SSW and AMU and a day off in lieu is provided. The speciality teams provide ward cover. Christmas has a separate rota.

Person Specification

Education, Experience & Training


  • Shall have full registration and a Licence to Practice with the General Medical Council
  • Shall have completed a minimum of 12 years' medical work (either continuous period or in aggregate) since obtaining a primary medical qualification of which a minimum of six years should have been in a relevant specialty in the Specialty Doctor and/or closed SAS grades. Equivalent years' experience in a relevant specialty from other medical grades including from overseas will also be accepted.

  • MRCP or Equivalent
  • FRCP or equivalent
