An exciting opportunity has arisen for an enthusiastic, motivated Band 7 paramedic to join our dynamic primary care team with Gosport central PCN. The successful applicant will work as part of the primary care service working a varied caseload in the same day access service as first point of contact as well as with our frailty and home visiting services.

We are committed to continued professional development and promoting evidence-based practise to ensure a high standard of care throughout the service. There is an extensive professional development programme aligned against the First Contact Practitioner roadmap to practice and advancing clinical practice. The successful candidate will have regular supervisions with their team lead. We offer a varied local in-service training programme as well as the opportunity to attend TARGET days. We are keen to develop our workforce and the individual will also have annual opportunities to apply for external course funding, including modules at MSc level in line with the service and the applicant personal development goals.

Main duties of the job

Gosport Central PCN has over 84,000 patients and this exciting role will be pivotal in supporting meaningful change and alignment of services within the PCN. The Paramedic role will provide a fantastic opportunity for a driven and passionate individual to drive innovation and support the ongoing excellent care of patients in the Gosport area. The Role will be supported by a GP prescribing Lead with the opportunity for extended Supervision across a wider PCN.

The post holder is an experienced paramedic who aspires to provide a responsive, safe, effective, and caring service to patients.

Role involves initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinic decision making and expert care, including assessment and diagnostic skills, for patients within the general practice setting.

Along with using a medical model to assess, diagnose and prescribe for acute medical conditions, the clinician will use a holistic approach to consider methods and interventions to prevent recurrence of problems and complete referrals to appropriate members of the wider multidisciplinary team.

The clinician will participate in, and influence efforts, across health and social services to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management and assure early intervention through proactive provision of care in or as close to the patients' own home as possible.

Job responsibilities

The post holder will be required to:

*Act as a role model in the promotion of person-centred practice, and challenge practice which is not person-centred, so that a person-centred culture is maintained

*Monitor, review and evaluate care interventions to ensure optimum quality is maintained

*Lead on the assessment, diagnosis, planning, implementation and evaluation of treatment/interventions and care for patients presenting with an undifferentiated diagnosis.

*Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.

*Assess, diagnose, plan, implement and evaluate interventions/ treatments for patients with complex needs.

*Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long term condition (as appropriate).

*Diagnose and manage both acute and chronic conditions, integrating both drug and non - drug based treatment methods into a management plan.

*Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence based practice and national and SHFT protocols, and within scope of practice.

*Work with patients in order to support compliance with and adherence to prescribed treatments.

*Provide information and advice on prescribed or over the counter medication on medication regimens, side effects and interactions.

*Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

*Inform clinical manager of any concerns regarding patients, relatives, visitors or staff that may compromise patients care

*Develop and maintain constructive working relationships and liaise effectively with all members of the multidisciplinary team so that patients' needs are met

*Responsible for planning team meetings, staff rosters and organisation of the team workload

Person Specification

Qualifications

Essential

  • Registered EC or Paramedic, or other relevant 1st Degree
  • * Master's degree in Advanced Clinical Practice or equivalent
  • * Active and relevant professional registration
  • * Evidence of relevant CPD
  • * Advanced Life Support Provider


Experience

Essential

  • A) is educated to degree/diploma level in Paramedicine or equivalent experience
  • B) is registered with the Health and Care Professions Council (HCPC)
  • C) has completed their two-year 'Consolidation of Learning' period as a & quot newly qualified paramedic& quot
  • D) has in depth experience as a band 6 (or equivalent) paramedic
