- Registered Nurse or equivalent health care professional
Desirable
- Evidence of CPD
- Ability to present and teach
KNOWLEDGE & EXPERIENCE
Essential
- Minimum of three years relevant post registration experience including working with Primary and secondary care and adult services
- Recent and significant experience of working with patients / carers/ MDT in preparing for discharge from hospital
- Experience of producing and assimilating report using a variety of data systems.
- Recent experience of working across primary and secondary care
- Specialist knowledge of the discharge to assess model and discharge pathways
- Knowledge of the Mental Capacity Act, Safeguarding Adults and Deprivation of Liberty.
