To deliver a high standard of therapy provision within the Urgent Community response/Virtual ward Team. To provide rehabilitation to our patients; promoting recovery, reducing risk of admission / re-admission and facilitating discharges and flow from the acute setting into the Community.

The Urgent Community Response Team aims to deliver holistic assessments and interventions to individuals who are at risk of hospital admission within their own home within 2 hours of referral to the service. The aim of the team is to enable people to stay at home, recover, and re-engage with the important aspects of their life and prevent admission to acute sectors.

The post holder will ensure access to Urgent Health Assessments, provide and monitor short term treatment programmes at home and ensure safe discharge or referral to long term health and social care interventions. Utilising clinical expertise, clinical judgement, discretion and decision making in the development of individualised treatment plans.

To work in partnership with primary, secondary and other health and social care providers, to ensure patients receive appropriate investigation, intervention and treatment with minimal avoidable delays, whilst improving the quality of care.

The post holder will be expected to work within a flexible working pattern over a 7 day week in the urgent community response team as required.

The post holder will work in the Urgent community response team and Virtual Wards to assist in the prevention of inappropriate hospital admissions and the facilitation of hospital discharge in line with the overall objectives of the service.

Person Specification

Qualifications

Essential

  • Good standard of general education including, GCSE English and Maths grade C or above
  • A Levels or equivalent qualification
  • Degree/diploma in Physiotherapy/Occupational Therapy
  • Registered with Health and Care Professionals Council
  • Evidence of CPD including recent post graduate training relevant to clinical speciality


Experience

Essential

  • Demonstrates clinical experience enabling the post holder to manage a caseload of patients with complex needs. This would usually be approximately 12 months and
  • will include experience in Medical, Older People, Trauma and Orthopaedics
  • Specialist knowledge across a range of practices, routine and non-routine, requiring expertise within the field of Physiotherapy/Occupational Therapy relating to Community Services
  • Previous and on-going attendance at mandatory training
  • CPD portfolio
  • Previous NHS experience
