Job summary

This is a fixed term/secondment vacancy until 01/07/2024.

If you are an internal applicant this post will only be offered on a secondment basis, and you will need to have gained your line managers approval for the secondment prior to going to interview.

A great opportunity has arisen to join the Community Nursing team in the Cardigan/Newcastle Emlyn area for 12 months, fixed term. We are looking for an enthusiastic nurse with a can do attitude, a forward thinker with a passion for nursing to join our friendly team.

The Community team is dedicated to support your continued professional development through various opportunities. The post will enhance your knowledge and open opportunities for further progression within our directorate.

Main duties of the job

You will possess excellent communication skills, enjoy working as part of a team, be able to work independently, carry out comprehensive assessments, plan, implement and evaluate delivery of complex nursing interventions as part of the holistic approach.

The successful candidate should have transferable skills that would support the team, assessments of CHC and Datix.

If you are dynamic, organised, work well within the multidisciplinary team and have an interest in caring for people in their own home with various chronic, long term conditions and end of life care, we would like to hear from you.

The ability and means to travel between patients in a timely manner is essential.

The ability to speak Welsh is desirable for this post. English and /or Welsh speakers are equally welcome to apply.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through:

Four main hospitals: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;

Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.

Person Specification

Qualifications and Knowledge

Essential

  • Level 1 Registered Nurse (Adult)
  • Current NMC Registration
  • Evidence of personal continuing development, statutory and mandatory training
  • Awareness of Health Board policies, procedures and guidelines
  • Ability to relate theory to practice
  • Understanding of primary health care working


Desirable

  • Post-registration experience, in acute services
  • Teaching and assessing
  • Understanding of clinical governance, health & safety, risk management awareness
  • Community experience
  • Willing to undertake further training as necessary
  • Evidence of post- registration education - Diploma / Degree Level


Experience

Essential

  • Ability to demonstrate sound knowledge of NMC Code (2018)
  • Able to relate theory to practice through reflective skills.
