We have a brand-new opportunity for a Band 5Mental Health Nurse, Social Worker or Occupational Therapistwith mental health experience to join theAcute Day Service, here at Langley Green Hospital.

The Acute Day Service offer service users in a mental health crisis, four weeks of psycho-educational groups to help learn coping strategies and self-manage their mental health.

Main duties of the job

As a Community Mental Health Practitioner in our team you will meet with service users provide a care plan, review their mental state, run the therapeutic groups. You will be responsible for monitoring and developing treatment plans in order to make progress with service users and ensure effectiveness of interventions.

You will be directly involved in developing discharge plans in collaboration with service users and their families, as well as occasionally representing the clinical team at multi-disciplinary meeting and clinical reviews of patient care.

You will work very closely with the CRHTT (Crisis Resolution & Home Treatment Team) and our inpatient wards.

Qualifications

Essential

  • Registered Nurse with current registration appropriate to the job role OR registered Occupational Therapist, Social worker or alternative with current registration appropriate to role


Knowledge/Experience

Essential

  • Experience of facilitating groups for mental health service users
  • Knowledge of MHA (1983/2007).
  • Knowledge and experience in group treatment evidence based practice
  • Child and Adult safe guarding procedures
  • Change management, organising and prioritising workloads
  • Demonstrate ability to enhance care through evidence based knowledge
  • Demonstrate professional development relevant to the care group
  • Demonstrates ability to assess and manage clinical risk
