Job summary

The post holder will be a key member of the Integrated Community Nursing Team in delivering a highly specialised clinical service to service users and the wider healthcare team - they will be integral to ensuring an efficient, effective, co-ordinated, and responsive high-quality service is delivered.

The post holder will work as a member of the Children's Complex Nursing Team to promote and create independence for children with bowel and bladder complaints.

The post holder will become a team member of a passionate, and dedicated Children's Continence team who work collaboratively with children, young people (CYPs) and their families to get the best outcomes for CYP's referred to the service. Working within the wider multi-disciplinary team and linking in with other teams within such as the children's community nursing service and wider 0-19 Health Visiting and Public Health Nursing teams.

Main duties of the job

  • Work independently as an autonomous practitioner with responsibility for carrying out specialist assessment, planning, implementation and evaluation of individuals with bladder and or bowel problems, in the community / clinic / home environments
  • Assist and advise nursing staff and other health care professionals in the assessment, treatment and management of patients
  • Will have experience of supporting and advising children, and their families, with Continence issues, and can work independently, as well as part of a team
  • Support band 6 paediatric advisor, in management of continence clinics around the Knowsley area
  • Participate in patient satisfaction reporting to improve patient care.
  • Maintain registration in line with professional bodies.
  • Provide support to team members holding responsibility for mentoring students.
  • Work with individuals, families, carers, communities, Primary Healthcare Teams and other agencies to undertake a comprehensive assessment of patients / service users in order to inform a personalised plan of care / intervention.
  • Formulate and deliver an individual treatment programme using holistic assessment, reasoning and treatment skills based on a sound knowledge of evidence-based practice and a range of treatment options.
  • Evaluate patient progress, reassess, and alter treatment programmes using an accepted/ appropriate range of techniques as required and utilising regular support and advice from senior clinicians.

  • The post holder will be an effective communicator.
  • Be able to Work collaboratively within the team and other stakeholders. This will include forms of verbal / written report writing, referrals to other agencies.
  • Communicate complex sensitive information effectively with all patient groups / service users / carers where there may be difficulties in accepting or understanding information.
  • Assess capacity, gain valid informed consent, and have the ability to work within a legal framework with patients / service users and carers who may have difficulties understanding complex information.
  • Maintain accurate, comprehensive, and up-to-date documentation in line with legal and departmental requirements and communicate assessment and treatment results to the appropriate disciplines in the form of reports and letters.
