East Lancashire Hospitals NHS Trust is looking to recruit a highly skilled and motivated Advanced Clinical Practitioner in Frailty. This is an exciting opportunity for a qualified ACP to join an expanding and motivated team and to be involved in the ongoing development our frailty services.
The post will be based mainly on our ACP-led Same Day Emergency Care Unit (Older Persons Rapid Assessment Unit), with in-reach to the Emergency Department as well as providing specialist frailty input to patients in other areas of the trust as needed. The post will include weekend working as required.
Main duties of the job
As a qualified ACP, you will provide leadership and clinical guidance to junior and non-specialist clinicians and other members of the multi-disciplinary team and be responsible for overseeing all aspects of the clinical care of frail elderly patients. You will be supported and encouraged to have an active role in service development, quality improvement and education within the department and across the wider trust.
What we offer
Our team is led by two Consultant ACPs, with support from Consultant Geriatricians, nursing leaders and other AHPs. The team have led significant changes to our frailty services over the past year, resulting in a high performing seven-day frailty service. We hope to recruit an enthusiastic ACP to contribute to our continued improvement.
We are keen to hear from qualified ACPs of any professional background with a keen interest in frailty and elderly care.

- Undertake clinical and holistic assessment of frail elderly patients in the Older Persons Rapid Assessment Unit and across other clinical areas as needed
- Provide guidance and expert advice to other members of staff to ensure safe and effective care for frail elderly patients
