The SMART Service is nurse led, therefore the ability to work autonomously as well as effectively within a team is essential. The work will include visiting patient in the community at the place of abode between Medway and Swale, therefore it is ESSENTIAL that you have a 'FULL UK DRIVING LICENSE AND OWN A CAR THAT CAN BE INSURED FOR BUSSINESS'.

Main duties of the job

As a Staff Nurse you will provide clinical and professional leadership, supervision and support to patients, and students within the SMART team.

You will triage, assess, plan and implement the care of patients who are discharged home under the care of the SMART service in the community, review and give care to SMART patients attending SDEC/ Hot clinic for review, administer IV treatments for Covid Positive patients at our CMDU, and conduct virtual visits using our remote patient monitoring pathways.

Members of SMART work autonomously, therefore we are looking for someone who is willing to lone work. Specific training will be provided to facilitate the pathways covered by SMART in-house as well as the community. Confidence, good communication skills and the ability to make sound decisions is also vital.

Job responsibilities

The SMART Service is nurse led, therefore the ability to work autonomously as well as effectively within a team is essential. The work will include visiting patient in the community therefore it is usually essential that you have a full driving license and a car but the CMDU is on site so a non-driver would be considered.

The Staff Nurse must also be able to liaise with all members of the multidisciplinary team to coordinate safe effective and timely care practices within the SMART service.

The successful post holders main responsibility will be the day to day review of referrals into the service, telephone triaging, clerking and preparing patients safely to be transferred into a virtual bed, conduct virtual visits via SMART remote monitoring dashboard, man the RPM dashboard between the hours of 8am to 8pm.

Support the CMDU, ensuring that all patients referred for treatment are triaged safely and the prescribed drug administered via IV. It is essential to have cannulation skills to deliver this pathway.

The Staff Nurse must also be able to care for OPAT (Outpatient parenteral antibiotic therapy) and post-operative patients. Full support and training will be provided to be able to support these patients safely.

As a SMART nurse teaching, auditing, infection control practices and undergoing research are activities undertaken within the role.

To apply you should be competent in cannulation, venepuncture, general surgical wound care, drain care. The service prides itself of being innovative, so an added vast expertise is welcomed. We are always very keen to expand the service further so any new ideas are welcomed.

You should also have access to a car and be able to drive to visit patients at home when required.

SMART will support with training to enable the successful candidate to meet the demanding and exciting challenges of the role. Person Specification

Qualification

Essential

  • RN1/RN2/RN12 or RN Child Registered Nurse on the NMC register
  • To hold the appropriate level for the area of speciality


Knowledge

Essential

  • Knowledge of the NMC Code of professional standards of practice and behaviour for nurses and midwives
  • Up to date knowledge of current clinical and professional issues


Experience

Essential

  • To participate in or provide evidence of completion of preceptorship
  • Is able to administer intravenous drug therapy/ or is willing to work towards this


Skills

Essential

  • Able to assess plan, implement and evaluate programmes of care
  • Able to supervise and teach junior staff
