Band 6 Chronic Kidney Disease nurse

This is an excellent opportunity for an individual to progress in their career and learn with an enthusiastic and committed team.

The post holder will provide high quality care and advice to patients and their families with Chronic Kidney Disease (CKD) within a clinic setting and in conjunction with members of the multidisciplinary team (MDT).

Main duties of the job

  • Play an integral role in the delivery of care to patients with chronic renal impairment.
  • Ensure continuing day to day provision of specialist care and guidance for pre-dialysis patients in North Cumbria in conjunction with the MDT.
  • Act in accordance with the NMC code of professional conduct.
  • Perform / request blood and radiological investigations where necessary. Analyse results and seek advice from medical staff, making recommendations for change in treatment where appropriate.
  • Clearly communicate the patient's chosen care pathway to the multidisciplinary team, enabling assured feedback, support and consistency in the patient's ongoing treatment.
  • Deliver patient care in clinics, visits and telephone communication. Provide home visits to known patients when requested.
  • Utilise communication skills when discussing patient's condition / treatment, outlining disease processes and consequences. Build up rapport with patients and their relatives, enabling difficult and complex issues to be discussed, making every effort to maintain understanding of condition and treatment plan, by providing relevant information, education and advice.
  • Inform patients about treatment options available, including the option of not dialysing and the implications of their choice. Support patients, carers and staff as needed, particularly those who have opted not to dialyse.
  • Assess and interpret conditions relating to the care of patients with chronic renal impairment and identify appropriate clinical situations requiring medical practitioner input.
  • Act as anaemia coordinator for the CKD population optimising iron and erythropoietin therapy as per agreed protocols.
  • Work with other multidisciplinary team members in delivering an evidence based approach to management of patients prior to dialysis.
  • Review patient's nutritional status with renal dietitians, referring as required

Person Specification

Qualifications

Essential

  • Registered Nurse with active registration.
  • Coaching and mentoring register or ENB136 /ENB998


Desirable

  • Further relevant postgraduate development at level 6 or above
