Job summary

An exciting opportunity has arisen for a Band 5 Community Staff Nurse to work in the Acute Response Team, based within the Intermediate Care service in Carmarthenshire. The successful candidate will be based within ART & the Intermediate Care Multi Disciplinary Team (ICMDT/Home first) at Eastgate offices in Llanelli, but working also from Glangwilli Hospital in Carmarthen.

You will be an integral part of ART & ICMDT and you will and will take part in the daily live triage of patients referred to Intermediate Care from both primary and secondary care with the main focus being \increasing patients time spent at home.

As part of this referral/ triage process and following acceptance of patients onto the caseload, you will be also be responsible for the Discharge to Assess (D2A) assessments of secondary care patients when discharged home.

The successful candidate will be a pivotal link between ART and the ICMDT and will work in both arms of the service.

Main duties of the job

The successful candidate will be responsible for providing clinical advice to the ART Team, ICMDT and other Professional agencies, acting as a resource for evidence based Intermediate Care.

The successful candidate will provide evidence based safe care to the Intermediate Care/Homefirst and ART Team in order to meet the needs of the caseload and local population and must be an experienced clinician.

The ability to travel between sites in a timely manner is essential for this post.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Qualifications and Knowledge

  • RGN Level 1 or 2
  • Current NMC Registration
  • Relevant post basic courses and/or experience
  • Mentorship
  • Evidence of continuing professional development
