Job summary

Are you an experienced clinical practitioner seeking a new challenge where you can make a positive difference? We are looking to appoint a Band 6 clinical practitioner to join our Learning Disabilities Assessment and Treatment Unit at Townend Court.

You will work in close partnership with adults with learning disabilities, their families, the multidisciplinary team, and relevant others to coordinate the transition and discharge plans for adults with a learning disability that are open to our service. You will help to inform the person's needs, liaising closely with the inpatient team, community team, family and others involved in the patients care.

The successful candidate will be enthusiastic, forward thinking and have experience of MDT working.

Main duties of the job

To oversee and coordinate the discharge plans for patients with a Learning Disability admitted to Inpatient Services.

To work alongside the patient, their family, their social worker and the rest of the MDT to assess the patient's post-discharge needs.

To collate the recommendations of own and other professional's assessments to provide clear recommendations for the social workers and commissioners regarding post-discharge needs

To be a central point of contact for families, social workers, commissioners, and other services regarding discharge plans

Support family contact whilst a patient is staying on the inpatient service.

To attend MDT meetings and core group planning meetings, ensuring good communication between all parties.

To coordinate transition plans, and to continue to be a point of contact for the first few weeks after discharge from the hospital.

Person Specification

Qualifications and Knowledge

Essential

  • Professional knowledge acquired through a degree supplemented by a post-graduate qualification or specialist training, experience, short courses equivalent to post-graduate diploma qualification
  • Full understanding of relevant policy, legislation, drivers and their application to clinical and service area, e.g. Mental Capacity Act, Social Inclusion


Desirable

  • Evidence of project work that has impacted on practice and demonstrates a higher level of communication/planning/change that impacts within own area of work/responsibility


Experience

Essential

  • Evidence of continuing professional development (CPD) relevant to the clinical area at specialist level of care
  • Evidence of development of specialist practice skills and able to demonstrate the impact of this on practice change/development
  • Evidence of promoting/supporting active user/carer involvement/participation


Desirable

  • Demonstrable experience of working in the specific field where the post is held

Essential

  • Specialist knowledge across range of procedures underpinned by theory.
  • Professional knowledge acquired through degree supplemented by diploma level specialist training, experience, short courses

  • Evidence of CPD or experiential learning at an advanced level e.g. Post graduate/Masters level
