Job summary

This is an exciting opportunity for an enthusiastic and motivated nurse to join our new dynamic Integrated Urgent Crises Response Service providing holistic assessments and identifying treatment needs that can be managed within a variety of community settings.

The multidisciplinary IUCRS team works within the wider Emergency Department of Colchester Hospital and Northeast Essex community services and identifies patients that can, once deemed as not requiring acute admission, remain at home or facilitate a rapid discharge into an alternative community setting.

The registered nurse will also identify patients requiring outpatient parenteral antibiotic therapy (OPAT) within the acute setting and support a seamless discharge to the community.

You should be competent and confident in the initiation, administration and teaching of IV therapy. You will be able to make autonomous decisions around patient suitability for this pathway and support patients to self-administer as well and liaise with multidisciplinary team as appropriate. Good communication, leadership and management skills are essential.

The nurse post holder will be working shifts and will be expected to work some weekends and bank holidays and nights. A car driver is essential.

Main duties of the job

You must hold a relevant qualification with community experience or other comparable/transferable skills.

You should have a good understanding and knowledge of relevant legislation and treatment methods used in this specialist field. You should be adaptable to change and embrace innovation.

The post holder will provide rapid specialised assessment and treatment to patients who may present with complex physical and psychological care needs and initiate services to safely avoid unnecessary hospital admission.

The post holder will also support the unplanned community nursing teams with the delivery of IV antibiotic therapy and all relevant nursing intervention within the community.

This new service has been developed using national evidence that illustrates the benefits of community antibiotic services, facilitating early discharge from the acute sector to a community or other appropriate setting, supporting the prevention of hospital admission.

The post holder will also support identifying patients suitable to be transferred from the acute hospital to the community hospital.
